A 2024 consensus report from the American Diabetes Association identified three key strategies: Haak emphasized that dietary adjustments are an effective method focusing on reducing fat and carbohydrate intake He highlighted the effectiveness of an initial 12-day “liver fast,” a protein-restricted diet that helps improve metabolic parameters such as two consecutive “oat days” during which only oats are consumed combining dietary interventions with glucagon-like peptide 1 receptor agonists (GLP-1 RAs) has yielded promising results Haak noted that timing is critical due to the potential for gastrointestinal side effects associated with GLP-1 RAs While bariatric surgery can be effective for obesity in T1D it carries a risk for complications and requires careful patient selection as well as psychological and medical follow-up Metformin is effective for glycemic control but is not indicated for T1D alone metformin can be used in patients with T1D and T2D “Metformin should definitely be included in the concept,” he said though it is not suitable for long-term use due to the risk for hypoglycemia The DDG guidelines highlight the role of metformin in managing insulin resistance in patients with T1D GLP-1 RAs can be used to manage obesity and insulin resistance caution is required due to the increased risk for diabetic ketoacidosis While these medications are not contraindicated their costs are not reimbursed in many healthcare systems Haak referenced findings from the DEPICT-1 study which showed that sodium-glucose cotransporter 2 (SGLT2) inhibitors in T1D can lower insulin requirements and reduce hypoglycemia risk but are also linked to ketoacidosis and other adverse events "SLGT2 inhibitors are contraindicated in T1D; Thomas Edelmann, 49, lives in Germany with his family, and he wanted to get in touch. Years ago, in 1938, Hanna’s grandfather was forced to sell his business far below the market price. Jews were being persecuted across Germany, and the 1938 Nazi Nuremburg Laws mandated Jews sell their properties or businesses to non-Jewish buyers buying companies and other assets from Jews at able to find Benjamin Heidelberger or his heirs Given the horrendous fate of most of Germany’s Jews it was entirely possible that he’d never survived Thomas was speaking with a salesman from the genealogy company MyHeritage which he’d used to help dig into his family history He mentioned his grandfather’s business and described how Benjamin Heidelberger was forced to sell it The salesman was intrigued and related the story to MyHeritage’s research department a representative from MyHeritage phoned Thomas back: they’d found records of Benjamin and Emma moving to Israel and even located their gravesites in northern Israel They told Thomas that their granddaughter Hanna Ehrenreich was still alive and encouraged him to send her a message through the company’s website he was finally speaking with Benjamin Heidelberger’s heir learning all about her family and hearing tales of his own grandfather as well She described how her grandfather felt gratitude towards his grandfather “Edelmann came to me and said I should leave Germany as quickly as possible There were plans in place to act against Jews and he felt obliged to warn me The Heidelbergers used the paltry proceeds they’d been paid for their property to flee the country for Israel, leaving Germany just weeks before Kristallnacht, the “Night of Broken Glass” anti-Jewish pogrom that consumed Germany and Austria on November 9 and 10 in 1938 the small Jewish community of Bad Mergentheim was terrorized was beaten up and the town’s synagogue was ruined All of the Jewish men in the town were arrested and sent to Dachau for instance – her mother’s parents – never made it out of Germany and were murdered by the Nazis Hanna told all this to Thomas and relayed that she’d even visited the store in the 1980s when she and her family took a trip to Germany and stopped in Bad Mergentheim which today is home to a chain of stores called Edelmann “I knew Edelmann was indeed the person who bought the shop,” she later explained although he was a member of the Nazi party.” “He was very moved and said he was so happy to hear the story from my side – he was almost crying,” Hanna described “It was such an emotional moment when I heard Hanna on the phone and when she told me about her grandfather,” Thomas said “Although her family was treated so badly she was very friendly and didn’t hold me responsible for anything.” Thomas still harbors doubts about his grandfather’s character “I know my grandfather was a very good businessman When he was a student during the 1920s he was already a member of the Nazi can tell at first glance that the two musicians Alexander Kilian and Jan Pascal harmonise well Neither tries to push himself to the fore and both speak with admiration of the other's work The musicians who form guitar duo Café del Mundo say that in the 16 years they have been performing together Both have pursued a career in classical music in 1987) began a classical music education which he completed with a jazz music diploma who actually had a penchant for punk and metal learned about Eastern music from his Georgian master He came to flamenco via the traditional music of Georgia - the path of a classical concert guitarist As he had always been fascinated by the sound of the flamenco guitar which he had learnt about during stays in Spain he finally followed "the call of Paco de Lucia" Kilian and Pascal met at a flamenco workshop in 2007 and knew immediately that they could work well together They stress that although they use the flamenco guitar they do not play flamenco in the true sense of the word but use the sound of the Spanish guitar to combine classical pop and flamenco to create a musical experience that appeals to a wide variety of audiences The two musicians describe one of their main aims as "to present the soulful primal forces inherent in the flamenco guitar to an international audience" They want to work on the evolution of an instrument that offers "possibilities of articulation" far beyond folklore They see the "fusion of different cultures as practised in Andalucía as a living example" as one of the "privileged tasks of music" whether their own compositions or versions of classical The fact that two such virtuoso artists can play together without their egos getting in the way is due to their differences: "Alex is more of a virtuoso soloist I'm more of a composer and the one who articulates the melodies," explains Pascal Kilian admires his colleague's compositional talent I know that it will bubble up from Jan and he knows exactly what works in my hands I don't think anyone else could write pieces for me as perfectly as he does," he enthuses Their fruitful collaboration is also helped by their shared musical vision and the goal of making international music with the sound of the flamenco guitar "but a positive one that helps us get the best out of each other," says Pascal The response from the audience is particularly satisfying for the two musicians when they see that the joie de vivre inherent in flamenco has spread to other countries "People leave the concert with a smile on their faces and you can see that they have been transported for a moment and have been able to leave their grey everyday lives behind," says Pascal they didn't want to perform at all at first as they felt it was inappropriate to present themselves as German musicians in the land of the flamenco guitar was convinced that their playing would go down well here too And ultimately Kilian believes that they also have something to contribute to flamenco as they are doing something completely new And this has earned them the respect of Spanish flamenco artists They have performed on stage with Spanish flamenco
