LBV Magazine English Edition Recent excavations uncovered a Roman monument at the Open-Air Museum of the Roman villa of Hechingen-Stein The discovery involved more than 100 fragments featuring depictions of ancient gods and figures from Roman mythology This archaeological treasure was publicly revealed on October 24 Klaus Kortüm of the Landesamt für Denkmalpflege (State Office for Monument Preservation) in Stuttgart the original monument was composed of several stacked stone blocks each decorated on all sides with reliefs illustrating deities and mythical characters from the Roman period leaving only fragments of the original figures visible Identifying these pieces has been made possible through comparisons with better-preserved finds Reconstructing this Roman monument has been a major challenge as only a small part of the original pieces has been recovered so far To restore it to its complete and proper form archaeologists have opted to use scaled-down replicas made with 3D printing allowing them to assemble a model faithful to the original structure Kortüm noted that the final plan is to display both the original fragments and the reconstructed model in the museum so that visitors can appreciate the scale and detail of this unique piece This discovery is particularly significant as divine-themed pillars or similar monuments are extremely rare in the Roman frontier provinces located between the Rhine and Danube rivers preliminary research suggests that the size of the Hechingen monument is unusually large compared to other examples found in Europe This raises the question: who might have been the wealthy owner of this property who commissioned or consecrated such a work and what was the reason behind this homage to the gods The discovery not only sheds light on the cultural and religious context of the ancient Roman villa in Hechingen-Stein but also reaffirms the importance of this site as one of the region’s most significant archaeological complexes The villa was originally excavated between 1978 and 1981 at which time the site was transformed into an open-air museum that has grown steadily Thanks to the collaboration between the LAD and the local association responsible for the museum new excavations have taken place nearly every year since 1992 allowing further exploration and discovery of life and economy in the Roman settlement museum visitors can appreciate the full scale of the Roman villa including agricultural buildings and the wall that surrounded the property along with exhibits and objects found stent in FEVAR procedures In what ways has the BeGraft as a bridging stent changed how you implant a fenestrated graft I think the BeGraft has changed our workflow and the ancillary tools that we are using We have almost exclusively changed to using 6Fr catheters to get into the target vessel whereas previously we were using 7Fr sheaths routinely The access you need from the iliacs is smaller with a 6Fr catheter If you use a pre-loaded renal fenestrated endograft you can access both renal arteries from the side of the endograft delivery system and then on the other side you only need to access the two visceral vessels In cases such as this we can use a 14Fr sheath where we might once have used a 20Fr or larger sheath It is risky to use large sheaths in the iliacs because they can cause rupture and so if we can avoid using them that makes the procedure safer What are the main features of a good bridging stent I think you need to combine a good endograft design with a good bridging stent in order to achieve positive outcomes using endografts with the fenestration positioned at the origin of the target vessel—meaning that there is no gap between the origin of target vessel and the fenestration—provides a very stable platform for BeGraft as a bridging stent the bridging stent needs to be 6Fr compatible so that you can easily and safely position it in the target vessel It also needs to be flexible so that it can adapt to the various forces that are exerted on the target vessel the first 20mm moves with the aorta and then the distal portion moves with respiratory motion and so a flexible stent is really important here if you want to avoid issues during follow-up it is necessary to have various diameters and lengths available so that you can adapt your approach to the patient anatomy and safely perform the procedure With the BeGraft you have a lot of options available in this regard How does the BeGraft compare to other bridging stents on the market and the 200 microns thickness of the ePTFE provides a secure coverage of the cobalt-chromium stent the BeGraft is perfectly suited for use as a bridging stent in FEVAR and we now have strong data to support this You conducted a study with the BeGraft as a bridging stent in FEVAR We were looking primarily at the patency of fenestrated endografts for short necks or juxtarenal aneurysms we were looking for things like type III endoleaks and stent fractures Three-year data from this study have been published Could you briefly summarise the main outcomes at this time point The patency was excellent and there were only two secondary procedures I think these outcomes are a result of good patient selection If you design the appropriate fenestrated endograft then the BeGraft will provide a perfect platform I think we had such a low number of technical issues because we perform a cone-beam computed tomography (CT) scan at the end of each procedure If you capture an issue at the end of the procedure with your cone-beam CT and then you do not need a secondary procedure Will there be further follow-up of these patients All our patients are in a prospective cohort and so we continue to collect data on these patients We have been using the BeGraft as our primary bridging stent for around seven years now and we have more than 500 fenestrated cases in our database There is also a dedicated prospective study currently running in Germany to evaluate the BeGraft as a bridging stent for fenestrated endografts It is great that data are coming from different centres to consolidate our findings You also performed a study on the movements of stents once they have been implanted Could you outline the key findings from this study and as mentioned in your previous question that there is some movement in all the target vessels during respiratory motion All patients enrolled in that study underwent a pre-operative CT scan in inspiratory and expiratory motion and then we did the same scan postoperatively with the endograft and the BeGraft in all target vessels and performed again inspiratory and expiratory acquisition of the CT scan We looked at how the bridging stent modified the way those visceral vessels behaved and it was very interesting to see that there was only a minimal impact We were actually not changing the way those target vessels behave Bentley is working on an on-label indication for the BeGraft in FEVAR To date we have all had to use the bridging stents in an off-label setting for FEVAR procedures and so I and other physicians are very pleased that an on-label indication is on its way Case images: Selective angiograms confirming patency and sealing after implantation of a BeGraft into the left renal artery during FEVAR and into the coeliac trunk during BEVAR Treatment of penetrating aortic ulcers (PAUs) has been swayed by recent advances in endovascular aneurysm repair (EVAR) the majority of diagnosed PAUs are treated by endovascular techniques rather than by open surgery and the minimally invasive approach is yielding encouraging results.1 In this arena the BeGraft aortic stent from Bentley (Hechingen Germany)—a balloon-expandable covered stent—shows promise addressing issues of “overtreatment,” says Nikolaos Tsilimparis director of vascular surgery at Ludwig-Maximilian University Hospital in Munich Tsilimparis has extensive experience treating PAUs and here speaks to Vascular News on how balloon-expandable covered stents have changed his approach to the indication treatment with large diameter covered grafts was “too much for too small a lesion,” whereas the BeGraft aortic is more tailored to the unique characteristics of a PAU thereby providing “the right tool” for this distinct pathology Before using balloon-expandable covered stents how did you treat patients with PAU disease we would treat PAU patients with either standard infrarenal devices iliac limbs (if the diameters were appropriate) we were overtreating a very localised lesion in the aorta We started to use balloon-expandable covered stents and noticed that the BeGraft aortic addresses the issue of overtreatment thanks to its ability to be dilated to different diameters allowing the physician to adjust the degree of therapy in order to match the extent of the disease one of the main challenges with this procedure was that standard infrarenal bifurcated grafts are designed to be used in large aneurysmatic aortas Aortas with PAUs often have diameters of between 14 and 18mm whereas self-expandable aortic stent grafts are made for large diameters of around 24mm Another issue was that an aortic aneurysm has more space to accommodate the bifurcation of the stent graft using iliac limbs of 12 or 16mm diameters in a narrow aortic bifurcation makes it very prone to occlusions These were both challenges that have now been eliminated thanks to the BeGraft aortic Which anatomical conditions are needed in order for a balloon-expandable covered stent to provide good outcomes A basic condition is the ability to reach a healthy aortic segment as balloon-expandable stent grafts are not very long but one has to be cautious not to cause a rupture with a balloon-expandable stent due to extreme oversizing of the lower part of the aorta What made you decide to try out the BeGraft aortic for this indication I always thought that we were doing too much for too small a lesion Drawing on my experience with this balloon-expandable stent in other indications and getting to know it as a very reliable stent graft that can be inserted with a low-profile system of 9–11Fr I thought it would be worth trying for PAUs we realised that it is an excellent solution for this indication What are the main advantages and limitations of using a balloon-expandable covered stent in a PAU I think the balloon-expandable covered stent offers very accurate positioning and also flexibility in terms of how much of the lesion you want to cover it has a big advantage in terms of the sizes that are on offer It must be taken into account that one needs to develop techniques for how to deal with the challenges of using balloon-expandable covered stents in the aorta We always use the lowest diameter of the aorta to start with and then we taper them instead of putting the bigger diameter stent into a smaller aorta We do not measure to the proximal or largest diameter What are the long-term outcomes in patients you have treated The long-term reliability of a balloon-expandable stent in an aorta that degenerates over time is a big concern the pathology is a little bit different with PAUs than with the typical atherosclerotic aortic aneurysm that will evolve and develop PAU patients have a very localised plaque rupture or have a local dissection—indications that require treatment but these patients still have a relatively stable aorta I would always follow these patients extensively especially at the beginning of our experience with a standard protocol that is similar to the one we use for infrarenal aortic aneurysms A multicentre study led by our institution found that use of the BeGraft aortic for the treatment of infrarenal PAU produces excellent results with low complications.2 How many patients do you see with a typical PAU and how often do you treat them with an endovascular approach The indications for treating PAUs are not yet well defined so while we are currently not treating a lot of patients I was previously very cautious about treating a patient with a small aortic ulcer or a local dissection by using an aortoiliac stent graft because I thought it to be an overtreatment it is just like putting a covered stent in the iliac arteries I now feel much more comfortable offering endovascular therapy to patients that present with these findings What are your key messages for treating PAU patients with this endovascular approach My main message is that we are able to treat a disease of its extent with the right tool We no longer overtreat just because we do not have the right tools there are many centres that have now decided to use this technique and it is interesting to note that different centres have developed different tactics and strategies in order to deal with or to treat these patients with the BeGraft aortic I think that it is an excellent tool and technique for this indication and I believe that the more we learn from our experience the better we will be able to understand the pathology and the further we will be able to Frederick the Great He blamed the otherwise unexplained discharge on an ‘implacable personal enemy’ Friedrich next found less-prestigious employment managing the royal household of Hohenzollern-Hechingen he met Benjamin Franklin who offered him work training American revolutionary troops — but no pay now Baron Von Steuben of Hohenzollern-Hechingen the baron returned home to rearranged the camp on Prussian lines Kitchens on the uphill side of the camp and toilets on the downhill He began training a model company of men who could in turn train others The Prussian trained the Americans to fight tactically discipline and fighting prowess showed immediate improvement Baron Von Steuben also socialised with the soldiers as he worked to raise their morale Pierre-Etienne Du Ponceau later wrote about a potluck dinner in his quarters his aids invited a number of young officers to dine at our quarters on condition that none should be admitted that had on a whole pair of breeches… Torn clothes were an indispensable requisite for admission… The Baron loved to speak of that dinner But it is unlikely the baron’s secretary would have documented the dinner party if it later descended into an orgy Not long after Baron Von Steuben’s arrival at Valley Forge the first ever discharge of an American soldier for homosexual acts occurred at the camp George Washington ordered Ensign Frederick Enslin drummed out of the camp for attempted sodomy with an 18-year-old subordinate But the law also required proof of emission (cum) or at least penetration the charge of attempted sodomy allowed for the prosecution of men caught in flagrante delicto but without visual or medical evidence of emission or penetration So Ensign Enslin at least escaped with his life even if subjected to the humiliating ritual documented by an officer in his diary “I this morning proceeded to the grand parade where I was a spectator to the drumming out of Lieut He was first drum’d from right to left of the parade thence to the left wing of the army; from that to the centre and lastly transported over the Schuylkill with orders never to be seen in Camp in the future This shocking scene was performed by all the drums and fifes in the army— the coat of the delinquent turned wrong side out.” It would seem extremely unlikely Von Steuben was ‘openly gay’ Rocking up for duty in a grand horse-drawn sleigh constantly stroking his miniature greyhound And those elaborate silk-lined uniforms bedecked with furs But Europeans were all a bit dandified compared to the rugged American colonials it stretches credulity that no one noticed the baron’s consistent choice of notably younger good-looking men as his aides And his affectionate relationships with those men But the revolutionary war effort benefitted greatly from Von Steuben’s efforts and the top brass liked the charismatic baron The baron became particularly close to Benjamin Walker and William North two officers in their 20s and conjectured by some to have been a couple even before meeting Von Steuben North named two of his children after Von Steuben who legally adopted the two men and made them his heirs another young man came into Von Steuben’s life John Mulligan met Baron Von Steuben soon after graduating from Colombia College in 1791 But the future president and his wife started to worry about the intense nature of the relationship between their son and young Mulligan The heartbroken couple wrote to Von Steuben who immediately replied with an offer to take them in “My heart and my arms are open to receive you In the midst of the attention and fetes which they have the goodness to give me I enjoy not a moment’s tranquillity until I hold you in my arms… “If our friend [Charles Adams] could accompany you with the same tender friendship I feel for you.” Charles Adams didn’t stay long but 19-year-old John Mulligan remained working as Von Steuben’s secretary until the baron’s without von Steuben—or that he should be considered a gay man by today’s standards I add new historical material to the growing list of details that support these claims These additions to his story could make von Steuben (1730-1794) the first “Don’t Ask Don’t Tell” case in the American military—quite possibly due to his own deeds and actions To appreciate the contributions von Steuben made to the American Revolution consider this: before his arrival in Valley Forge in 1778 the Revolutionary Army had lost several battles to Great Britain the United States of America might still be one of the British colonies the Revolutionary Army was a loosely organized rag-tag band of men with little military training The military fumbled through the beginning of the war for independence George Washington and the Continental Congress knew that without help from additional seasoned military experts Since Washington himself was the best the colonies had they looked to Europe for someone who could train the troops Washington wrote to the colonies’ representative in Paris was treated as a celebrity in the French court This would be pivotal in achieving his two major objectives in France: winning financial support for the revolution and finding military leaders who could bring a semblance of order to the Revolutionary Army Franklin learned of a “brilliant” Prussian military genius Von Steuben had a string of success stories (some of them self-embellished) with the Prussian army But there was one problem: he’d been asked to depart because of his “affections for members of his own
