Through a partnership with the South Kitsap Helpline the clinic is delivering nutritious foods directly to patients with diabetes who are also facing food insecurity and mobility challenges brought the pilot project to Kitsap in September The model has been implemented at several VMFH facilities focusing on cardiovascular diseases and diabetes It began last year at a heart failure clinic in Tacoma before expanding to other cardiology and diabetes clinics across Pierce County The Kitsap pilot is part of a broader VMFH push to reduce food insecurity through a variety of new programs $500,000 grant from CommonSpirit Health Mission and Ministry Fund VMFH reports it has 139 patients enrolled in the food box program and has seen a 61% reduction in hospital admissions and 75% reduction in emergency room visits among enrolled participants the project supports distribution of food boxes to patients every other week over a six month period The program is serving about a dozen patients so far and could grow to as many as 50 Kropp-Wolner said the program is not “standard medicine” but providing healthy food to people with chronic disease like diabetes can help them meet prescribed diet goals and prevent additional complications from a poor diet patients with diabetes who have low incomes or mobility issues face challenges accessing healthy foods Whole and nutritious Ready-to-go Azure security services further free up the small IT staff Stiftung Diakoniewerk Kropp uses an Azure Firewall to create and enforce security policies block traffic to and from problematic IP addresses and prevent malware and viruses from taking hold “We had to answer the big question of how we could ensure security now and in the future,” Schnack says “We outsource a lot of functions to this technology giving my team the flexibility to concentrate on improving processes that benefit our organization and the people we serve IT issues are significantly simplified with the help of Microsoft.”  it should come as no surprise that the COVID-19 pandemic accelerated this healthcare nonprofit’s digital transformation What may be unexpected: Stiftung Diakoniewerk Kropp’s staff even those who do not consider themselves IT-savvy have enthusiastically embraced Microsoft productivity Each organization within Stiftung Diakoniewerk Kropp has its own team keep their files organized in a SharePoint intranet “They log in just once and have everything they need in one interface They immediately feel comfortable with the familiar experience and even have fun exchanging information,”
