requires technological solutions to effectively meet the needs of a growing patient population to enhance the delivery of genomic results and related clinical information to patients and families GUÍA development occurred in five overlapping phases: formative research Development was informed by formative qualitative research involving parents (N = 22) whose children underwent genomic testing Participants enrolled in the NYCKidSeq pilot study (N = 18) completed structured feedback interviews post–result disclosure using GUÍA and community stakeholders provided their perspectives on GUÍA’s design to ensure technical NYCKidSeq participants responded positively to the use of GUÍA to deliver their children’s results All participants (N = 10) with previous experience with genetic testing felt GUÍA improved result disclosure and 17 (94%) participants said the content was clear GUÍA communicates complex genomic information in an understandable and personalized manner Initial piloting demonstrated GUÍA’s utility for families enrolled in the NYCKidSeq pilot study Findings from the NYCKidSeq clinical trial will provide insight into GUÍA’s effectiveness in communicating results among diverse genetic counselors (GCs) are challenged to scale services to meet the needs of a wide array of providers Outcomes from these studies will help to inform best practices for delivering genomic medicine services GUÍA development occurred in five overlapping phases over a 16-month period Development phases included a formative qualitative study of parents (N = 22) whose child had undergone genomic testing (phase 1) content development guided by an expert working group (phase 2) recurring feedback from stakeholders and community members (N = 11) (phase 3) and development and testing of the web application (phase 5) of 22 index children who had undergone exome sequencing or microarray clinical genetic testing within the previous 12 months to participate in an in-depth interview Participants were recruited at two major health systems in New York City: Mount Sinai Health System and Montefiore Medical Center We used a stratified purposive sampling approach to recruit a diverse group of participants: five Black/African American (AA) and two who self-reported as more than one race or ethnicity (MR) We also stratified the sample to ensure representation of different testing results: ten negative Interviews were administered in English or Spanish Parents received transportation costs and a $50 gift card for participating The Expert Working Group (EWG) was formed with transdisciplinary expertise in the development of educational and medical genetics content The EWG led the development of the overall design of GUÍA and the development of the content and final versions of visuals and text throughout the development process the EWG and a UI designer collaborated to produce a final set of wireframes (page schematics) and visual designs which were used as the basis for the personalized result templates A bilingual version of each wireframe was generated that displayed both English (small font) and Spanish text (large font) The only text not translated to Spanish was the navigation button as this was not a priority for the research study We aimed to ensure that the Spanish text was understandable to patients who speak different dialects of Spanish the translation team included individuals representing five Spanish dialects: Spanish Discrepancies in the translated text were discussed and resolved and UI designer developed a specification for GUÍA development and established the display logic and the data flow The back-end database contains a web form through which the GC inputs patient specific information to populate the patient-facing front-end application The database includes a library of template text including over 5,000 variables and approximately 50 illustrations that are displayed based on patient-specific results A team of software engineers built the application over a 3-month period followed by a 4-week user acceptance testing period during which the EWG performed extensive UI testing to evaluate UI interactions before the platform went live GCs built a participant’s case in GUÍA by inputting case details Case details were input as both structured and free text Structured text can be displayed in both English and Spanish Spanish free text was generated using Google Translate and Spanish-speaking staff reviewed the translations for accuracy GCs used GUÍA via an iPad during the in-person results disclosure session Families did not have access to GUÍA outside of the counseling session Families were provided with a PDF printout of the full GUÍA content in addition to the laboratory report Feedback was solicited from four GCs regarding their experience using GUÍA during the pilot phase structured feedback guide to explore parent-participants’ reactions to GUÍA The guide contained 14 questions designed to capture general and specific reactions to GUÍA content and design elements Research coordinators interviewed parents in English or Spanish directly following disclosure of their child’s genomic results using GUÍA Feedback interviews were audio-recorded and parent-participants were provided with a $40 gift card A separate research coordinator reviewed the audio recordings and extracted structured and narrative details of the parent-participants’ responses These responses were reviewed for themes relating to the GUÍA interface The formative study comprised 22 interviews with parents of children who had received genomic results within the previous 12 months, sampling for diversity in both child race/ethnicity and type of genomic result. Seven core themes were identified, which are listed in Table 1 with participant quotes exemplifying each theme Parents’ primary concern was improving health outcomes for their child parents were more interested in understanding how genetic results affected their child’s care than understanding technical details We therefore designed GUÍA to prominently display follow-up care recommendations on the Next Steps page Parents expressed confusion about the labels of genetic test results negative results were sometimes described as “normal” and were confused with the child’s symptoms GUÍA clearly defines the result category (positive negative) on both the Learn about Genome Sequencing page and the Result Summary page Another theme identified was that parents who received uncertain results adapted to the uncertainty when the provider explained that genomic medicine is a developing science and knowledge of the genetic cause of disease will increase over time We included language addressing this in GUÍA drawing complex concepts and showing illustrations to parents were appreciated Parents expressed the desire to take home drawings made by a provider to revisit with family members illustrations are integrated throughout the application The EWG first identified discrete components of a typical pediatric genetic counseling result disclosure session, as shown in the schema in Supplementary Fig. 1 GUÍA addresses the following components: genetic education primary (related to the primary indication for testing) and secondary (unrelated to the primary indication for testing) results disclosure Contracting and psychosocial counseling are addressed through the interpersonal interaction between the counselor and the patient/family We designed GUÍA to have a user-friendly interface with a visual design that is meant to make information communicated during the result disclosure easier to understand GUÍA enables GCs to guide patients through the distinct components of a traditional result disclosure session while catering to patients’ diverse learning styles Using GUÍA can be a counselor- or patient-driven experience and its modular design permits users to control the order in which information is accessed the user can follow a proposed linear flow by using the navigation arrows at the bottom of each page or they can navigate to different pages of the application from the Home page or the navigation panel The user can toggle between Spanish/English or English only text by clicking the language button Illustrations accompanied by plain language text are incorporated throughout the application to convey key concepts and demonstrate aspects specific to the patient’s condition Images show the GUÍA Home page displayed in English only (a) the Result Summary page for a positive result displayed in Spanish/English (b) and the Family page for a positive result displayed in Spanish/English (c) Eighteen NYCKidSeq pilot phase parent-participants completed feedback interviews. All 18 were female; mean age was 44 years (range 28–56 years). Two (11%) were AA, 10 (56%) were H/L, 5 (28%) were EA, and 1 (6%) was MA (Table 2) Six (33%) interviews were conducted in Spanish Six (33%) parent-participants’ children received positive genetic results GUÍA was used by both the parent-participant and counselor to guide the delivery of information parent-participants were asked which component they would like to focus on next none of the sessions were reported as parent-driven as all parent-participants when given the option to explore GUÍA on their own deferred to the GC to walk them through the tool In the feedback interview, parent-participants’ general reactions to GUÍA were overwhelmingly positive (Table 3) parent-participants felt that GUÍA made receiving information about the result manageable and they appreciated being able to read along as the counselor discussed the results Spanish-speaking parent-participants valued reading text in their preferred language All parent-participants (N = 10) who had previous experience with genetic testing agreed that using GUÍA improved result disclosure Seventeen (94%) parent-participants said the content was clear and all parent-participants stated that the amount of information contained in GUÍA was the right amount Parent-participants appreciated that they could control the flow and amount of information provided to them Seventeen (94%) parent-participants felt that the user interface was easy to navigate One parent-participant suggested including a “back” button on every page to better enable the user to navigate between pages of the application While the majority of parent-participants felt that the number of illustrations included in GUÍA was the right amount and that the illustrations helped them understand complex genetic concepts (15 [83%] one parent-participant stated that including additional illustrations would be helpful for individuals with limited understanding of genetics GCs report spending approximately 15 minutes inputting data for each case When asked to describe their experience using GUÍA compared with traditional counseling tools (e.g. GCs highlighted the advantage of having information displayed in both Spanish and English when counseling a Spanish-speaking family using a telephone interpreter They also expressed that GUÍA provided GCs the ability to personalize patients’ results saved time in developing patient educational material and made it easier to communicate results with other providers Limitations of GUÍA were noted as being restricted to GS tests children could disrupt the session by their interest in the iPad patients/families do not have access to an online version of the tool and GUÍA does not support annotation on pages likely due to the amount of education typically occurring during the initial appointment and are not designed to improve patient understanding of their genetic results Findings from this preliminary evaluation suggest that GUÍA may be well positioned to support the growth of applied GS in that it facilitated result disclosure and was favorably reviewed by participants and GCs alike The pilot evaluation of GUÍA described here was carried out in a small which might limit the generalizability of the findings Since the cohort consisted of participants in an ongoing study their reflections on GUÍA may have been influenced by pretest counseling familiarity with the genetic testing process and repeated interactions with study staff This study demonstrates the benefits of the development of technological solutions for communicating genomic information by medical professionals; going to see each other next year I didn’t understand what next year meant.” Calling from New York was expensive in the days before cell phones “We had a lot of quick calls,” Fatima recalled April was 3 months old when Guia brought her back to the Philippines All four girls would follow their mother into nursing “We grew up with not enough money,” Fatima said Mama Guia’s daughters followed her into nursing Guia’s annual summer visits allowed her to attend graduations but that meant she missed the holiday season “She’s never home for Christmas or New Year,” Fatima said Fatima did not spend the holidays with her mother until she was 21 and had also immigrated to New York as a nurse “There’s always a big celebration there.”  The emergency room was one of the busiest in the city and could be especially wild during the 8 p.m And it was very clear who was running things ‘I can’t believe she’s holding down the emergency room by herself,” Fatima remembered Fatima understood that the people who called her mom Mama Guia were as dear to her as if they were her kids Her family there included down-on-their-luck alcoholics who were regular night visitors “You’re here again,” Guia would say to them “You can’t leave drunk people in the street.” Fatima said of her mother’s approach to nursing Fatima understood that at the core of it was the same generosity of spirit her mother showed off-duty “She would always think about other people first,” Fatima said “She would put others first wherever she goes Fatima went to work in the emergency room at another Brooklyn hospital “It’s been a dream of mine to be like her,” Faitma said Fatima proved to be very much her mother's daughter as COVID-19 suddenly began filling the hospital “It was tough for everyone to see patients die left and right.” adamant that she did not want to be intubated “She knew the odds once you were on a ventilator were low She spoke to Nguyen-Phuc through an oxygen mask I’m going to figure it out,’” Nguyen-Phuc recalled who spoke to The Daily Beast with Fatima’s permission had met Guia when also working at New York Community The doctor knew that even someone as determined as this legendary head nurse could reach a limit “I had a lot of concerns she was getting tired and might not be able to make it through,”  Nguyen-Phuc recalled “There’s only so much you can do on strength of will alone She had already been doing it for three weeks Nguyen-Phuc also knew that COVID-19 could take a sudden turn for the irretrievable worst I didn't want to risk her crashing and dying in front of me,” Nguyen-Phuc later said The doctor advised Guia that if they were going to have to intubate they should do it in the most controlled way possible “She acknowledged that it seemed inevitable,” Nguyen-Phuc remembered “She looked at me and she looked at the nurse and said Nguyen-Phuc and the nurse most definitely did not screw up Her daughters were able to talk to her on speaker phone as she lay unconscious She died on April 26 despite everybody’s best efforts “Thank you for trusting me to take care of you, Guia,” Nguyen-Phuc wrote  afterwards in a private GoFundMe appeal for Guia’s family Seeing your ferocity and This will automatically set a variety of application and viewer settings Feel free to try other options or make your own template File → Select - This is where you select what mesh you would like to display and paint your textures on using a 1 meter by 1 meter cube mesh with just 6 faces (no subdivisions) will give you an exact preview of how your textures will look when mapped onto a block in the game Document Resolution - This is the texture resolution that you’re targeting Note: This setting can be changed later non-destructively at any time it’s recommended to work at 1024 - you’ll get lots of detail in your textures with minimal pixelation the app will run nice and fast on most PC setups and your HD textures should look sharp in the game If you’ve only ever worked with textures in vanilla Minecraft you’ll notice that you can’t select 16 pixels by 16 pixels here for your resolution (the lowest is 128) you can always author and export textures at 128x128 (or larger) and scale them down later in an image-editing application like Photoshop Normal Map Format = DirectX (leave as default) Compute Tangent Space Per Fragment = True (leave as default) Add - (Optional) Here you can choose to import any supporting textures that you already have completed If replacing a vanilla Minecraft texture with an HD version it can be helpful to import the vanilla texture here for use as a reference Note: This import step can also be performed later at any time the vanilla “iron_block” texture file is going to be imported when the new project is created to be used as a reference The application should open this new project that you’re creating and look something like this: It’s a good idea to add the emissive map to your texture set list at this point Even if your texture set isn’t going to use emissivity it will default to black (0.0) and won’t hurt anything Note: You can also add an opacity map to your texture set list in the same way if you’re planning on creating a non-fully opaque texture set add your texture reference to a new fill layer in the Base Color slot You’ll find your imported reference texture on the Shelf under the Project heading Layers are automatically set to bilinear filtering when created which can make pixelated textures like vanilla Minecraft textures look blurry leave your layer filtering on the default setting of bilinear HQ it’s also a good idea to set this reference layer to have full roughness (1.0) This will make sure that the reference texture is displayed in Painter as close to the vanilla game as it can be Note: If you didn’t import reference textures or other resources when you created the project you can do so at any time using the Import Resources button on the Shelf It’s recommended to name your Painter project after the game object whose texture set you are replacing this project file has been saved as “iron_block.SPP” Naming your project this way will make things easier when exporting your texture set later you can create your texture set with all of its PBR maps as you see fit a new fill layer was added (with some adjustment sub-layers) to create a dark a new fill layer adding an indent detail to give the surface of the block some new character another fill layer to add some dirt collecting in the corners of the indent and some scratches randomly across the surface Now that the whole texture set is looking great inside Painter          Select: File → Export Textures to open the export subwindow Create a new export preset configuration like this which will output your texture set in a way that can be used by the game build Note that the “$project” prefix on each output map will name each of the exported texture files after the name of the project if you named your project after the game object that this texture set will belong to all of your exported textures will immediately be ready for use in the game without any additional renaming select it in the “Config” field and use these recommended settings: Location - This can be the textures location for the resource pack that you’re working on This makes things easier because you can export textures from Painter and they will immediately show up or be updated in the same folder that the game is referencing when it runs (assuming that you have your resource pack activated in the game) PNG -
