I think she can definitely help us going forward." Night-time calls for updates on Breya and the latest on the family home her parents recently put on the market have helped Tia slowly regain her footing If life in a family of athletes brought pressure it also gifted Tia with a certain perspective Tia has understood the long game this spring there's a pathway to regular playing time and likely multiple trips to the WCWS Recent history suggests there's a good chance she'll lift a national championship trophy before she leaves Norman achieving something neither of Tia's sisters ever did none of that diminishes the inescapable pressure of her last name "You can't run away from it," Lawyer emphasized just like it will Breya; the last Milloy sister is a rising eighth grader and herself a promising softball prospect It's why Lawyer was so proud when his third daughter chose to go to Oklahoma Tia paid close attention while the Sooners cultivated status as the standard across college softball during Amirah and Kiki's respective careers When the time came for Tia to embark on hers Lawyer watched his daughter identify her own mountain to climb in the challenge of competing with the very best at Oklahoma "Her journey is different from all of ours," Lawyer said If she handles all of this in the next few years At Cornell, loneliness is a pervasive issue. Social platforms like Sidechat regularly feature posts from students expressing their struggles with feeling like they have no friends or support system. These posts garner much sympathy and empathy from other students who relate to the feeling. And here’s our email: associate-editor@cornellsun.com I didn’t realize until after my recent visit to Tia’s Tacos that the Turkey Creek taqueria was essentially a reincarnation of Paco’s Tacos the owners had some legal issues with the former name forcing them to rebrand under the current moniker Even though Tia’s (Spanish for “aunt”) is in the same location as Paco’s so I went into Tia’s Tacos under the impression it was a brand-new entity I’ll tell you how I think this new venture stacks up to the original Tacos indeed are the main attraction, but the menu offers several chips-based starters, with toppings like salsa, cheese dip and house-made guacamole. The Grub Spouse and I weren’t interested in chips, so I forged ahead to the lineup of street tacos. Having since gone back and re-read my original review of Paco’s Tacos I noticed that the selection was virtually the same Then you choose either a corn or flour tortilla (or the tortilla of the day) you decide whether you want it topped off street style (onions cilantro and lime) or American style (lettuce I requested three tacos as follows: grilled cactus with pico in a jalapeño corn tortilla (their special of the day) served street style ($4.54); chorizo in a flour tortilla street style ($4.29); and sauteed mahi-mahi with pico in a flour tortilla Other entrée options include Paco’s nachos (a nod to the restaurant’s previous name) 12 different signature burritos and a handful of sandwiches a 12-inch flour tortilla filled with melted cheese The Spouse went with grilled chicken ($10.59) We also ordered a couple of pieces of tres leches cake ($5 each) I snagged a couple of samples from the five-flavor salsa bar but when an employee showed up with our trays of food I was definitely ready for some south-of-the-border grub My sampling of the fajita-quesadilla was positive The grilled chicken embodied a flavorfully smoky essence The grilled and folded tortilla was basic in appearance but delivered more than expected in the eating The one with the least impact for me was the grilled cactus with pico which was dominated by the tortilla and its blend of jalapeño zip prominent corn flavor and unmistakable cilantro which played well with the milder flour tortilla and American-style toppings Taking top honors in my book was the chorizo taco and its seasoned sausage The spicy meat was complemented but not overwhelmed by the other ingredients we both really enjoyed our slices of tres leches cake which were moist and boasted an abundance of sweetened milk topping our Paco’s Tacos meal consisted of a specialty burrito I don’t know if the food quality overall has improved or if the difference in our orders accounts for the uptick But suffice it to say that if you get super-hungry for eclectic and tasty street tacos Bottom line: This locally owned Turkey Creek eatery serves a unique lineup of tacos First-time transient ischemic attacks (TIAs) may be associated with a long-term cognitive decline on par with stroke survivors potentially overturning conventional beliefs about the neurologic impacts of TIAs These findings suggest that healthcare providers should be conducting cognitive screening in patients who have experienced their first TIA an assistant professor at The University of Alabama at Birmingham Heersink School of Medicine “Stroke increases the risk of cognitive decline and dementia; however, compared with stroke, the relationship between TIA and vascular cognitive impairment is less well understood,” the investigators wrote in the February 10 issue of JAMA Neurology Del Bene and colleagues conducted a secondary analysis of data from 16,203 participants in the Reasons for Geographic and Racial Differences in Stroke study The cohort included 356 individuals who experienced a first-time TIA 965 individuals who had a first-time stroke and 14,882 asymptomatic control individuals Cognitive function was assessed every 2 years using verbal fluency and memory tests including the Consortium to Establish a Registry for Alzheimer’s Disease Word List Learning The primary outcome was a standardized cognitive composite score with secondary outcomes assessing specific domains of cognitive function To distinguish the effects of TIA from preexisting cognitive decline or other vascular risk factors the investigators used segmented regression models These models allowed them to compare the cognitive trajectories of individuals before and after their cerebrovascular event and vascular risk factors such as hypertension participants exhibited cognitive trajectories like those exhibited by asymptomatic control individuals the rate of cognitive decline accelerated significantly The annual cognitive decline rate among patients with TIA vs control individuals was −0.05 vs −0.02 SDs per year (P = .02) the decline observed in the TIA group was not statistically different from the decline seen in stroke survivors who experienced a cognitive deterioration rate of −0.04 per year (P = .43) The cognitive impairment observed in patients with TIA was primarily driven by declines in immediate and delayed memory recall This suggests that TIAs may selectively impact memory-related brain regions though the underlying mechanisms remain uncertain In an accompanying editorial an assistant professor at Weill Cornell Medicine argue that these findings challenge the traditional notion that TIAs are benign events with no lasting consequences “TIAs are far from benign and should instead be considered potential harbingers of progressive cognitive decline,” they wrote Their editorial noted that this new study addresses many of the limitations of previous research by using a large and rigorous adjudication of TIA and stroke events the exact mechanisms driving post-TIA cognitive decline remain poorly understood “The authors speculate that beta-amyloid and tau pathology could be present that gamma-aminobutyric acid transmission could have been disrupted or that blood-brain barrier opening could have promoted neuroinflammation,” Smith and Navi noted “a similar phenomenon of accelerated cognitive decline has been seen after systemic vascular events This raises the possibility of other mechanisms These findings have immediate implications for clinicians managing patients with TIAs Given the elevated risk for cognitive decline physicians should not view TIAs as isolated transient events but rather as markers of potential long-term neurologic impairment “Stroke-prevention specialists and primary care practitioners should question patients with TIAs about the presence of cognitive symptoms and be prepared to do a cognitive screen for impairment,” they recommended “TIA should now be viewed as a risk marker for cognitive decline as well as a risk marker for adverse vascular events.” Smith and Navi stressed the need for further research to elucidate the mechanisms linking TIA to cognitive impairment Possible avenues of investigation include examining neuroinflammatory markers assessing subtle structural brain changes using advanced imaging and exploring potential interactions with neurodegenerative processes Understanding these mechanisms could inform future therapeutic strategies aimed at reducing post-TIA cognitive decline The investigators reported funding from the National Institute of Neurological Disorders and Stroke and the McKnight Brain Research Foundation Navi reported receiving personal fees from MindRhythm Send comments and
