This wonderful tool helps people with a tracheostomy improve their vocal communication by redirecting “airflow through the vocal folds

if my neck is bent too far forward while using the PMV
creating a life-threatening emergency for me
I remember well when I started using the PMV at age 3
I had received my tracheostomy a year earlier and was using a ventilator
I was introduced to the Passy Muir Valve as I was being weaned off the ventilator during daytime hours
but it would take me quite a while to get used to the device
and caregivers using a PMV for the first time is that patience is crucial for all parties involved
everybody should educate themselves on the device as much as possible before using it
The PMV provides an opportunity for caregivers to form a trusting relationship with the patient during the adaptation process. The caregiving team must put themselves in the patient’s shoes
When I first started using the Passy Muir Valve
it felt like my airway was blocked because I didn’t know how to breathe with the device
Whenever I practiced breathing with the PMV
I had someone right there who could take it off quickly when needed
but it took months for me to adjust to breathing with the PMV
You can’t tell someone to just “get used to it.” It’s not that simple
Again, trust is required. The patient needs the gift of reassurance that they will be closely monitored while wearing and getting used to the PMV
They need to know that a caregiver will take care of their breathing needs
one thing that helped me get used to breathing and talking with the PMV was having a distraction
I was home-schooled during preschool and much of kindergarten
I had improved enough with the Passy Muir Valve that I could at least say short answers verbally
someone would place the PMV on my tracheostomy so that I could answer
and then the PMV would be taken right back off
One day I became so engaged with my lesson that I didn’t even notice that the device had been on for 30 minutes
At night I would go back on the ventilator
My advice for getting used to the PMV is to do whatever it takes to distract yourself when it’s on
Other people can help by engaging in fun activities with you
breathing and talking with the Passy Muir Valve will probably become like second nature
Please remember that these experiences are told through my eyes and perspective
Some may be able to handle the PMV instantly
Others may be unable to handle it due to an occlusion or narrowing of the tracheal walls
I don’t want to make it seem like getting used to the PMV is a near impossible obstacle and not worth all the time and effort
What I gained from using the Passy Muir Valve far outweighed the difficulty of adapting
it brought me a treasure load of friendships over the years
Those friendships have resulted in memories that add a rich texture to my life
As I said in a previous column, I now use my TD I-Series computerized voice device in addition to the PMV
They both are great tools that allow me to use my real voice as well as my computerized voice
In short, the Passy Muir Valve has helped me zoom up many of the mountains I face with SMA and allowed me to accomplish many goals in life. Being at the top of a mountain has a way of bringing you not only the world, but also a world of hope

BELFAST (WVII) -- We've all heard of Punxsutawney Phil, but have you heard of Passy Pete?
Instead of using a shadow to forecast spring, a little Maine twist is put on the tradition.
For the past nine years, residents of Belfast have relied on Pete the lobster to determine if it will be an early winter or if summer will last a little bit longer.
Much like Groundhog Day, the ceremony involves Pete choosing a scroll with a prediction that is read to the community.
This year, it’s not the result residents were looking for, but they aren't surprised.
“We're very sorry to share that he predicted an early winter this year. It's cold here in Maine and you get used to it. And if you wear the right clothes, you'll be fine. And I imagine that prediction is always going to be cold,” said Ken Morris, a Belfast resident.
If Passy Pete is right, we have to enjoy this warm weather while we have it.
Pascaline Peter, known as Passy Ma Trevor on Facebook, was arrested on Thursday in connection with a major fraud scheme that has left hundreds of Kenyans devastated.
Pascaline is the suspect in a case involving the alleged embezzlement of an estimated Sh350 million from unsuspecting investors.
According to the police, Passy Ma Trevor deceived her victims by offering a fake real estate investment deal through her company, Pafrim Investments Ltd.
In an elaborate scam, Pascaline had convinced victims to make deposits running into millions of shillings in four bank accounts spread across Equity Bank, Coop Bank, Stanbic Bank, and Kingdom Bank.
She promised returns of up to 30 per cent monthly and falsely claimed to be running successful a real estate company as bait. Once the payments were made, she switched off her mobile phone and went into hiding.
Detectives from the Directorate of Criminal Investigations (DCI) and their colleagues from the Kasarani division tracked Pascaline to a hideout in Kantafu, Machakos County, where she was taken into custody.
The arrest came after an investigation was launched due to many complaints from people who had been scammed.
“Preliminary investigations have revealed that the 36 year old lady who displayed opulence across her social media outlets managed to defraud over 200 individuals, with 106 formal complaints already filed against her,” DCI said on Thursday.
Police said they are working to gather watertight evidence to enable her prosecution, urging the public to “remain vigilant and cautious of similar fraudulent schemes to avoid further financial losses in the hands of such individuals or companies.”

Biologists at The University of Texas at Arlington have published a study showing that if humans continue with business as usual regarding actions that cause climate change
many economically important species could be driven to the brink of extinction within 50 years
The study was conducted by researchers in the lab of Sophia Passy
was lead author and was joined by co-authors Chad Larson
a stream ecologist with the Environmental Assessment Program at Washington State Department of Ecology in Lacey
Pound and Larson earned doctoral degrees from UTA with Passy as their faculty advisor
The article, titled “Current distributions and future climate‐driven changes in diatoms, insects and fish in U.S. streams,” was published in the January edition of the journal Global Ecology and Biogeography
Its message is clear: The future of stream ecosystems depends on efforts to address climate change today
The study assesses the potential impacts of climate change on the diversity and distribution of 336 freshwater species across 1,227 streams in the continental United States
The team developed species distribution models for using water chemistry
Four climate change scenarios were examined: a mitigated scenario where carbon dioxide emissions decline to zero; two stabilizing scenarios where emissions peak and then decline; and a worst-case scenario where emissions continue to increase through the end of the century
referred to as “business as usual.”
“Our results indicate a widespread decrease in cold-water taxa
projected to decline most drastically in localities where they are most abundant today
Given that such localities serve as important sources for colonists
the loss of cold-water species there is likely to have a negative impact at a regional scale,” Passy said
with projections showing a loss of more than 80 percent of their current distribution under the business as usual scenario,” Pound said
“We observed an increase in a number of warm-water and environmentally tolerant taxa
warm-water taxa were projected to nearly double their distribution in the most extreme climate change scenario
This is highly problematic because many of these species have characteristics of environmental concern
“Minnows are common bait bucket releases and hold great potential for introduction throughout their full forecasted range,” Pound said
“These seemingly harmless feeder fish are threats because they often hybridize with closely related minnows
diluting the gene pool of native species.”
Other species of concern include predacious longnose gar and flathead catfish
and high fecundity fish such as gizzard shad which dominate fish assemblages and outgrow predators
The study’s results also suggest diminished distributions of functionally important taxa within diatom and insect communities
“Results for diatoms suggest a decline in both understory and overstory taxa
affecting stream food chains,” Pound said
all aquatic insects classified as environmentally sensitive were cold-water
These insects are indicators of healthy streams but may be eliminated by climate change.”
Algae and insects are important food sources for fish
so dramatic restructuring of their communities could diminish overall stream biomass
Widespread increase of warm-water taxa and eradication of cold-water taxa may also lead to biotic homogenization
a phenomenon in which biological communities become more similar to each other
Biotic homogenization reduces the integrity of ecosystems making them more vulnerable to both natural and human disturbance
Given that the magnitude of these biotic shifts depends on the severity of climate change
appropriate current policy decisions are necessary to preserve freshwater ecosystems

I suspect it's the reason so many South Floridians learn to avoid the beach — it's almost a defense mechanism so we don't lapse into a state of Margaritaville-minded stupor
There's also this: South Florida is a place still very much defining itself — aside from tourism
our chief industry is sprawl (or these days
a place that's too busy expanding to figure out what it wants to be once it reaches capacity

when she said "there's no 'there' there," but is there any place more "there-less" than Palm Beach County
I spent much of my time in Palm Beach County searching for the "big city" within it
I liked to write about those institutions that "fought the good fight," as I put it
and tried to bring a measure of quality or an appreciation for the cutting-edge to the local cultural scene
Think Palm Beach Dramaworks and its intensely gripping approach to theater
Think the now-shuttered (but hopefully soon-to-reopen) Bamboo Room and its classy lineup of blues
Think the now disbanded Klein Dance and Ballet Florida and their relentlessly creative approaches to dance
which dared to program the occasional contemporary-minded artist in its Rinker Playhouse
My same hunger for the new and unusual applied — literally — when I changed beats and became a food writer and restaurant critic for the paper
I celebrated restaurants that were willing to look past South Florida's reputation as the prime-rib capital of the Western world
that meant ethnic eateries that proudly showcased their cuisine without dumbing it down: I was thrilled to find Chinese restaurants of the quality of Grand Lake Chinese Cuisine in West Palm Beach and Uncle Joe's in Palm Beach Gardens
that meant fine-dining establishments that brought a degree of sophistication and an appreciation for unusual ingredients or preparations
I still remember my first dinner at Dolce de Palma in West Palm Beach
where chef Anthony de Palma's contemporary take on rustic Italian cuisine wowed me
David Manero's new restaurant in downtown Delray Beach
where crowds happily devour a very modern bar menu
I also came to realize that I had it wrong
but the real pleasures of paradise are the ones ultimately more home-grown
I began to establish roots here and discover what was truly in my back yard — or close by to it
perhaps the most unsung suburban community in the United States
good rec programs and the best damn mall (The Gardens) on the planet
After bemoaning my lack of close friends in Florida
I found something better — neighbors and colleagues worthy of calling soulmates
I stayed up late in the evening with the couples down the block — Rick and Ruth
Mark and Paula — as we sat in each other driveways discussing nothing and everything all at once
a retired New York media type who read my articles and began to offer advice and critiques that always inspired me to do better
and though I never quite found the beach (at least on a regular basis)
I found plenty else to love about Florida: that long
those overly generous Amish restaurants in Sarasota
that old-timey thrill that comes whenever I walk down Main Street in Disney World's Magic Kingdom with my beloved wife and two kids
I found the annual funfest that is the South Florida Fair.)
I also found a great place in which to practice my craft — namely
But South Florida never was home — at least not in the deepest sense
Perhaps it's a game I played with myself — the grass was always greener (or the concrete always more solid) in my native New York
it's just that New York is my true home — not so much because it's the "big city" (though it certainly is)
but because it's where my parents still live and where most of my childhood friends still remain
It's not that it's "better." It's just that it feels right — at least for now

Aspiration is a common complication after tracheostomy in patients with acquired brain injury (ABI), resulting from impaired swallowing function, and which may lead to aspiration pneumonia. The Passy-Muir Tracheostomy and Ventilator Swallowing and Speaking Valve (PMV) has been used to enable voice and reduce aspiration; however, its mechanism is unclear. This study aimed to investigate the mechanisms underlying the beneficial effects of PMV intervention on the prevention of aspiration.

A three-dimensional (3D) upper airway anatomical reconstruction was made based on computed tomography scan data
followed by computational fluid dynamics (CFD) simulation analysis to detect subglottic pressure
Results: The results showed that compared with the non-PMV intervention group
the velopharynx maximal pressure (VP-Max) and upper esophageal sphincter relaxation duration (UES-RD) increased significantly (P < 0.05)
while the Penetration-Aspiration Scale (PAS) score decreased in the PMV intervention group (P < 0.05)
the subglottic pressure was successfully detected by CFD simulation analysis
and increased significantly after 2 weeks in the PMV intervention group compared to the non-PMV intervention group (P < 0.001)
indicating that the subglottic pressure could be remodeled through PMV intervention
Conclusion: Our findings demonstrated that PMV could improve VP-Max
and reduce aspiration in tracheostomized patients
and the putative mechanism may involve the subglottic pressure

This study aimed to investigate the subglottic pressure and swallowing biomechanics of aspiration after tracheostomy in patients with ABI treated with PMV and to explore the underlying mechanism of aspiration prevention
Tracheostomized patients with aspiration following ABI were enrolled in this study. The inclusion criteria were as follows: (1) diagnosis of ABI; (2) age between 18 and 80 years; (3) ABI occurs within 1–12 months; (4) use of a tracheostomy tube; (5) non-ventilated; (6) without supplementary oxygen requirements; (7) clinical symptoms of aspiration [Penetration-Aspiration Scale (PAS) score ≥ 5, Figure 1]; and (8) no medical history of previous dysphagia
The exclusion criteria were as follows: (1) unstable vital signs; (2) severe cognitive impairment; (3) inability to achieve an upright sitting position; (4) medical history of seizures; (5) allergy to iohexol injections; (6) pregnant or nursing women; and (7) any neuropsychiatric comorbidity or affective disorder that may influence the test outcomes
Written informed consent was obtained from all the participants or their relatives prior to inclusion
Aspiration occurred during swallowing in tracheostomized patients following ABI during VFSS examination

Thirty tracheostomized patients with aspiration following ABI were evaluated for eligibility, of which 22 patients who met the inclusion criteria were included and equally randomized into two groups. Two patients in the non-PMV intervention group discontinued the treatment. Therefore, 11 patients in the PMV intervention group and 9 patients in the non-PMV intervention group completed the study. Figure 2 shows a flow diagram of the inclusion process of the study

All data were obtained from the Third Affiliated Hospital of Sun Yat-sen University
The patients were divided into two groups by a random allocation sequence generated by a computer random number generator
All assessments were conducted in a single (assessor) blind manner
The PMV intervention group received a PMV intervention combined with conventional treatment
Oral cleaning and sputum suction were performed before the PMV intervention
The patient remained in a semi-recumbent position and the PMV was secured to the tracheostomy tube
Patients in this group also received conventional therapies for 30 min (once daily
These therapies included effortful swallowing
oropharyngeal muscle strengthening exercises
and postural compensation for head rotation
Patients in the non-PMV intervention group received the same conventional therapies as those in the PMV intervention group

The device used the proprietary pressure transduction technology (TactArray) which allowed each of the 36 pressure sensing elements to detect pressure over a length of 2.5 mm in each of the 12 circumferentially dispersed sectors

The subjects were placed in a neutral sitting position under the guidance of a C-arm remote control twin-perspective gastrointestinal X-ray machine (Toshiba DBA-300
Each subject was instructed to swallow 5 mL iohexol injection for a total of three swallows
The severity of laryngeal penetration during swallowing was measured using the 8-point PAS
A digital acquisition and analysis system for video fluoroscopy was used to quantitatively analyze the video fluoroscopic swallow study (VFSS) video

Patients were examined with PMV after treatment in the HRM and VFSS procedure in the PMV intervention group and the tracheal cuff was deflated during the examinations
whereas the secondary outcomes included subglottic pressure
The sample size was calculated such that at least nine individuals from each group had to be included in this study for 80% power with a 5% type I error level to detect a significant difference in PAS score (Park et al., 2013)
All statistical analyses were performed using SPSS 23.0 (SPSS Inc.
Data are presented as mean ± standard deviation (SD)
Categorical variables are presented as frequencies
Normally distributed data were determined using the Shapiro-Wilk test
Homogeneity of variance was measured using the Levene’s test
and presence of pulmonary infection were expressed as the number of participants and analyzed using Fisher’s exact test
Continuous variables of the baseline characteristics between the groups were expressed as mean ± SD and were analyzed using the two-tailed two independent samples t-test or Wilcoxon signed-rank test
The two-tailed paired samples t-test or Wilcoxon signed-rank test was used for changes in parameters prior to and after treatment in the PMV and non-PMV intervention groups
These tests were also used to compare the differences between the two groups before and after treatment (post-pre treatment)
The level of statistical significance was set at P < 0.05
The characteristics of patients in the two groups are displayed in Table 1. The biomechanical characteristics of swallowing in the two groups at baseline are shown in Table 2
There were no significant differences between the two groups at baseline in terms of age

Swallowing biomechanical characteristics and subglottic pressure in PMV and non-PMV intervention groups at baseline
To observe the effect of the PMV intervention, the swallowing biomechanical parameters were compared between the PMV intervention and non-PMV intervention groups. The representation figure of the VFSS is shown in Figure 1. Swallowing biomechanics were measured using HRM prior to and 2 weeks after PMV intervention (Figure 3)
The results demonstrated that VP-Max and UES-RD increased and PAS scores decreased significantly in the PMV intervention group compared to the non-PMV intervention group (P < 0.05). However, there were no statistical differences in TB-Max, UES-RP, LVC, AHD, or UHD (P > 0.05) (Figures 5A–H)
These results indicate that PMV can improve VP-Max

the biomechanical mechanism of aspiration prevention by PMV was explored in tracheostomized patients following ABI
compared with the non-PMV intervention group
and subglottic pressure significantly increased
while PAS scores decreased in the PMV intervention group
We concluded that PMV intervention could improve aspiration and remodel subglottic pressure after tracheostomy in patients with ABI
the subglottic pressure was detected with a relatively high accuracy in our study
We speculated that remodeling of the subglottic pressure by PMV intervention was a key factor in preventing aspiration
The above study demonstrated a correlation between VP-Max and aspiration; however
non-tracheostomized patients were included
The increase in VP-Max and UES-RD might be a protective factor for the prevention of aspiration
which were used to assess penetration and aspiration
The improvement of VP-Max and UES-RD combined with the increase in subglottic pressure after PMV seems to have restored the normal swallowing biomechanical parameters
which might explain the mechanism of aspiration prevention and improvement
we can conclude that aspiration is controlled by the sensory system to some extent
We further speculated that remodeling of subglottic pressure in the PMV intervention group might be due to preserving subglottic flow and producing breathing airflow with the help of the PMV
indicating that the UES might be controlled by the swallowing cortex rather than the periphery
The benefits of PMV and other one-way valves for speech and reducing aspiration risk must be carefully weighed against the above risk
especially for speech-language pathologists who have significant exposure to mucosal surfaces and secretions
As tracheostomy is usually be followed by long periods of functional dependency and rehabilitation
the importance of multidisciplinary team-based approach should be valued and prospective data are needed to further guide best practices in tracheostomy care during the current pandemic
long-term follow-up on the effect of PMV intervention and the effect of PMV on aspiration pneumonia is lacking and still needs to be explored in future studies
whether aspiration after tracheostomy in patients with ABI is due to changes in tracheostomy or brain damage caused by late neuronal dysfunction is uncertain
which also requires a no-aspiration group after tracheostomy in ABI for an in-depth
although the proximal cause of tracheotomy status in the patient population was ABI
this diagnosis and indication does not exclude the possibility of other concurrent pathology
we have not performed other diagnostic procedures
to evaluate the structural anatomic factors that might have contributed to need for tracheostomy or increased aspiration risk
other covariate factors related to aspiration were not controlled for
Further research is required to control for these confounding factors
and aspiration in tracheostomized patients could be improved by PMV
the benefits of PMV and other one-way valves for speech and reducing aspiration risk must be carefully weighed against the risk
such as aerosol generation in patients who may have risks of viral transmission
The importance of multidisciplinary team-based approach should be valued and prospective data are needed to further guide best practices in tracheostomy care
