but she was first and foremost a mother and a protector And she was baby obsessed from even a young age Maybe that's why she started having kids as young as she did "She went into the role of mother young and went into it with everything she had," he said Niki Capaci died last year at age 40 while incarcerated at the Orange County Jail in Goshen "Feeling lost is an understatement for us," her three youngest children said in a statement to NPR "We would have never thought that we'd lose our own mom so early in life Nothing will ever make us feel okay again because we won't ever see our mother again." The family has also been burdened with questions since her death more than a year ago: What exactly happened between May 3 when she was found unresponsive in her cell and was later pronounced dead The family is suing Orange County and the jail's health care provider in federal court for damages for personal injuries The lawsuit names two staffers: Cyrel Dasraj a nurse with Wellpath responsible for caring for inmates at the Orange County jail The lawsuit alleges that Dasraj failed to check on Capaci while she was in her cell and that Washington as well as other unidentified medical providers at the jail failed to properly assess and treat Capaci Neither could be immediately reached for comment Capaci's case draws attention to the challenge facing the U.S of caring for jailed people with addictions who are struggling with withdrawal Jails and prisons are frequently ill equipped to handle the medical needs required to safely wean someone off addictive substances like opioids At least 63% of people sentenced to jail meet the definition of addiction — compared with 5% of adults who are not incarcerated, according to U.S. Justice Department research That research also has found that among sentenced jail inmates with addiction issues less than 20% had participated in any form of drug treatment while incarcerated "and only 1 percent received detoxification services." The Capaci family has demanded information since Niki Capaci died but family members say the details they've received through records requests have only raised more questions about how she was treated by medical providers and what led to her death It has prevented them from properly grieving and moving on "It has dragged out the grieving process for all involved for way longer than it should It's been a year already," said Capaci's mother Capaci was being held in the Orange County Jail for violating her probation for prior drug charges The jail is owned and operated by the county and its sheriff's office Capaci had told her family that she was likely going to stay for no more than a week before probably being transferred to a rehab facility the family told NPR over a video chat in late May Capaci was exhibiting symptoms of severe withdrawal from opioids The lawsuit says that Capaci was intolerant to buprenorphine a common medication used to treat opioid use disorders who kept in touch with Capaci through calls while she was incarcerated She was also given a host of other medications according to her autopsy report and some medical records from her time in jail that were reviewed by NPR she became extremely ill with uncontrollable vomiting and diarrhea Besides being given anti-nausea and antidiarrheal medication and being checked by jail staff in her cell sporadically the family says it believes Capaci was largely left alone in her cell without any medical intervention by jail staff The family received the autopsy report for Capaci but it didn't include the fact that she died in jail It lists her cause of death as an accident caused by acute drug intoxication The autopsy report also makes no mention of the administration of buprenorphine that the family says Capaci told them she was given and that is listed as being given to her in medical records from her time in the jail The autopsy report also lists multiple medications — including a drug used for both anxiety and alcohol withdrawal (which Capaci had no history of being addicted to according to her family) and several benzodiazepines (which are used as a sedative) — that were found in her body The fact that this autopsy report doesn't list Capaci's death as taking place in jail is problematic but not unusual which investigates the history and current practices of the American medical examiner system "I see a lack of care to the amount of drugs that she was prescribed and its effects on her body," he said And because the autopsy report doesn't mention that she was in jail "this record reads like the state is trying to release itself from being responsible for her death," said Keel who is also a professor of human biology and society at the University of California "you would just simply say this person was a drug addict who died of drug complications but the context changes if you begin to realize that this person was in jail." All of this leaves the family with the belief that Capaci died from a combination of opioid withdrawal — not entirely uncommon according to experts who study addiction — and from medical neglect from Orange County jailers and the jail's medical staff the largest commercial provider of health care in jails and prisons across the United States Wellpath has been the target of several federal lawsuits and is being scrutinized by 12 U.S senators who are looking into its health care practices NPR attempted to contact the Orange County medical examiner who completed Capaci's autopsy We were referred to the office of Orange County Executive Steven M Representatives for Wellpath didn't respond to multiple requests for comment A representative for the Orange County Sheriff's Office said in a statement that Capaci's estate "has engaged legal counsel to file a Notice of Claim on its behalf A hearing was conducted with a representative of the Estate County Attorney Richard Golden has stated: 'The Estate of Nicki [sic] Capaci has chosen the litigation path to address its concerns the County's responses will be set out in that forum and will have no other comment.'" a member of the board of the American Society of Addiction Medicine the director of the National Drug and Alcohol Research Centre at Australia's University of New South Wales warn that failing to treat withdrawal symptoms in a safe and timely manner could lead to serious injury or death But finding explicit data on the prevalence of deaths in jail caused by drug or alcohol withdrawal is difficult Many of these deaths end up being categorized as death by "illness" or "other" by jails that submit death records to the U.S That's because there is no specific reporting category for deaths associated with withdrawal Jails, where there are typically not enough resources, "present challenges for addressing substance withdrawal" and addiction, the DOJ's Office of Justice Programs said in a June 2023 publication providing guidelines for local governments The guidelines recommend that jails and other institutions provide access to both buprenorphine and methadone considered front-line treatments for opioid withdrawal said it's crucial that jails and their staffs are properly trained to identify and treat withdrawal because it's a jail's responsibility to care for the people it incarcerates someone suffering from opioid use disorder who is incarcerated should have access to all necessary medications "Some patients may respond well to buprenorphine Other patients may respond better to methadone," particularly with opioids like fentanyl The Capaci family alleges that Niki Capaci suffered medical neglect during her severe withdrawal and says Wellpath is partly to blame established in its current form following a 2018 merger is the country's largest correctional health care organization providing medical services in prisons and jails for more than 300,000 people across 37 states Wellpath has been accused by critics of inadequate and delayed care with chronic understaffing and medical conditions going ignored which in some cases has allegedly led to inmate deaths the company has been sued by dozens of family members of deceased prisoners — now to include the Capaci family Some of these lawsuits have also targeted local jails or prisons and the towns and cities they are located in — costing localities millions of dollars in settlements and judgments from the lawsuits towns and cities across the country continue to choose Wellpath to provide medical care for their jails and prisons A 2019 CNN investigation found that in lawsuits filed between 2014 and 2018 was accused of contributing to more than 70 jail deaths Wellpath's predecessor, CCS, was named as a defendant in roughly 1,400 federal lawsuits over the decade leading up to the merger, according to a letter from 11 Democratic U.S Bernie Sanders that was sent to Wellpath and founders of H.I.G Many of those lawsuits have ended in some form of compensation for the plaintiffs Wellpath's leadership called deaths of inmates isolated incidents and defended the company's overall work who are leading the effort to question Wellpath Like the case being brought by the Capaci family some of the other lawsuits filed against Wellpath have alleged that employees ignored signs of distress from inmates In 2022, an 18-year-old named Dezaree Archuleta who was incarcerated in Colorado's El Paso County Jail died by suicide Her family told Colorado Public Radio that she had repeatedly expressed suicidal ideations The El Paso County Sheriff's Office settled with the Archuleta family for $1 million after the teen's death A 2021 Justice Department investigation of the San Luis Obispo County Jail in California found that Wellpath didn't have enough medical staff on duty The senators who have questioned the company have asked Wellpath specifically about its procedures to treat opioid withdrawal The deaths of two men incarcerated at New York's Orange County Jail before Capaci arrived also have been tied to poor medical care by Wellpath The 2021 suicide of 26-year-old Troy Conklin in the jail was investigated by New York's Commission of Correction. The agency found that he had a heightened risk for suicide but that Wellpath providers in the jail failed to recognize it Less than a year later, the commission found that the death of Ricky Mack, a 65-year-old man with dehydration, chronic kidney disease, COVID-19 and a host of other ailments at the time of his death, was tied to "gross failures in the medical assessments" of Mack. Had he been properly diagnosed and transferred to a hospital, his death could have been prevented, the commission's report concluded Wellpath responded in each report to say it was making improvements and holding more employee training The Commission of Correction has not commented on whether Capaci's death will be investigated Capaci stayed in school and went on to become a registered nurse at 21 This job allowed her to support her growing family "She did everything she could to provide for them She did a very good job — until the drugs," said Ben Capaci But the heavy lifting and tough work as a nurse quickly wore down Niki Capaci's body she injured her back and went on to have at least three surgeries for herniated disks Like so many other people in the U.S. who have an opioid addiction Capaci started out with painkillers legally prescribed to her by doctors while not realizing she was essentially a functioning addict leading Capaci to do what so many others do: find another source She turned to illegal forms of opioids and eventually fentanyl said their relationship was not always the best "Because of especially how the last few years went with me and my mom's relationship Davila was working at a Mexican restaurant It was Cinco de Mayo and the place was packed She was telling Davila that she was really sick Davila told Capaci to call the emergency button placed in her cell to get guards to help her and I heard them tell her to stop and [that] she wasn't dying Like I could tell she was puking on the phone Anybody with half a mind could tell it was serious." That was the last time she would speak to her mother Capaci was found unresponsive in her cell at approximately 6:04 a.m according to her family's lawsuit and jail medical records It wasn't until more than seven hours later that Davila was informed by Orange County Sheriff's Office officers that her mother was dead the family obtained some of Capaci's jail medical records compared the process of trying to get information to trying to put a puzzle together without all the pieces When Capaci was booked and placed in the jail's general population she was also placed on 15-minute interval special watches by jail and Wellpath personnel to monitor her for potential withdrawal symptoms But those checks took place less frequently and not in accordance with procedure At one point during Capaci’s incarceration two hours elapsed between checks by staff — not 15 minutes as required During the evening of May 5 and into the morning of May 6 there were stretches from 30 minutes up to an hour when no one looked in on her logs meant for tracking inmate behavior and special watches from May 4 at 3:30 p.m "are missing or non-existent," the lawsuit says "Even when the 15-minute checks were performed often reduced to a single glance into the jail cell," the lawsuit says The records they have received include contradictory and confusing information she was transported to the jail's medical center in a wheelchair and was given anti-nausea medication to help stop her repeated vomiting The suit alleges that no doctor physically examined Capaci and solely an "on-call provider" spoke to staff members over the phone and told them to give her medication to stop the vomiting Capaci was transferred again to the jail's medical center Jail surveillance videos of her awaiting transfer that were shared with NPR show Capaci barely able to remain standing upright in one area of the jail and leaning on the door she lies on the floor as she waits for a guard to open the door and attempts to administer CPR and an AED (automated external defibrillator) proved fruitless Farrell, of the National Drug and Alcohol Research Centre in Australia, along with two other professors of drug research, was part of a study titled "Yes, people can die from opiate withdrawal." It found that persistent vomiting and diarrhea are tied to untreated withdrawal The toxicology report included in the Orange County medical examiner's autopsy lists benzodiazepines the withdrawal drug Capaci was allegedly given while in jail who is not involved in this lawsuit but who reviewed Capaci's autopsy "There are high levels of controlled substances that are in her system that seem higher than what you would expect from someone being in jail for two days." There's "an extraordinary number of benzos," which are basically a sedative they can interact with one another" and with other drugs in the system the report shows that "either she was getting access to drugs while she was in jail Or they weren't giving her the drugs to deal with the withdrawals." "You got seven kids that don't really know what happened to their mother My sister went through a lot of excruciating pain There have been increased efforts in recent years to boost medication-assisted treatment in jails and prisons across the country Kathy Hochul signed into law legislation that establishes inmate access to medication-assisted treatment programs It also requires jails to have access to medication used to treat opioid use disorder The New York State Sheriffs' Association opposed the legislation, saying in a memo that the "decision whether to offer [medication-assisted treatment] is one that resides, and should remain with, the Sheriff," New York Focus The association also cited the cost of the medications and fears that inmates would attempt to smuggle opioids into jail the American Society of Addiction Medicine board member said having all medications available to incarcerated people can save lives "People with opioid use disorder who are on methadone or buprenorphine have at least a 50% decrease in mortality by being on the medication It decreases the risk for both opioid-related or overdose mortality," she said Under this 2021 law, local correctional facilities are required to submit an annual report of provided treatment services to New York's Commission of Correction The most recent report shows that the Orange County Jail couldn't initiate methadone therapy but otherwise "provided continuation for individuals already prescribed." Ramsey notes that efforts to provide particular medications for opioid use disorder in jails and prisons are slowly improving but part of that is due to a reactive response to lawsuits filed by families or federal agencies the Capaci family hopes that no other family goes through what they have And I don't want to see any other families go through this," Morse said "And if we can help get a little bit of policy and procedure changed or even adhered to … if anything can get changed from this lawsuit Become an NPR sponsor When Anna Capaci asked to make the eight-hour trip to visit the University of Dayton had one condition: she had to get her mileage in It was on that run that she decided UD would be her home for the next four years “As I ran through campus and the student neighborhood for the first time I was welcomed by smiles from strangers and greeted by students on porches,” Capaci said “It gave me a glimpse of the community I would be joining.” Capaci dove headfirst into the vast range of opportunities UD offers students opportunities she never thought would be possible opened up to her “Because of the scholarships I received toward tuition it opened up paths for me to be able to consider a summer study abroad,” she said She traveled outside of the country for the first time to Rome for a summer with a group of University of Dayton professors and students through the Business in Rome program expanded her perspective and changed her approach to conversations with potential future employers.  “I know that a lot of big public accounting offices have programs where you can go abroad to work at their overseas offices That’s definitely something that I’d be interested in that I wouldn’t have thought about before,” she said “So those conversations with different companies help me narrow down my job search.” In addition to her study abroad experience scholarships also gave her the opportunity to intern in Washington for one of the top six accounting firms in the country unexpected possibilities opened up for me that I never came to UD thinking that I was going to be able to do,” she said “There are so many opportunities offered at the University of Dayton and I truly could not see myself at any other university,” she said She credits her ability to attend UD to the generosity of donors “I would not be able to be in this position and I probably wouldn't have considered Dayton without the initial support from donors and scholarships,” she shared And Capaci still loves to run through the UD neighborhood: “The feeling that I got that day running through campus has not been dulled by living here the past four years and that is because the University of Dayton is truly so special.” Matthew Himes came to the University of Dayton to study business and after two years he found himself at the intersection of his academic and spiritual calling a company that provides healthcare services to Orange County Jail inmates recently announced that it was filing for bankruptcy and undergoing a “reorganization” of its correctional healthcare division As part of its Chapter 11 bankruptcy filing including the one filed by the family of Niki Capaci The lawsuit accuses the county jail and Wellpath staff of “deliberate indifference to serious medical need” and “medical malpractice,” and alleges that both Wellpath and Orange County are liable for Niki’s death said that the judge’s stay order extends to Orange County and its employees Oh said the bankruptcy filing has also disrupted the discovery phase of the lawsuit which includes the disclosure of evidence and other key documents for the case the family is frustrated by the inability to get the answers they were looking for and the indefinite delays,” Oh said in an email exchange we filed an objection to the stay being extended to the county and its employee as well as Wellpath’s individual nurse employee (as she’s not filing for bankruptcy individually) Hopefully we will receive the decision in the next few weeks.” which reported on the bankruptcy proceedings last week “I’m more frustrated about the fact that I feel like [Wellpath is] kind of trying to shirk responsibility I don’t care as much about my day in court or the money I care about there being some accountability we f****d up.’ And I feel like this is kind of a way for them to avoid having to do that.” Niki was arrested for a probation violation and sent to Orange County Jail the 40-year-old mother of seven died in jail The official complaint filed in 2024 with the U.S District Court for the Southern District of New York details Niki’s last days which resulted in her needing to be monitored for opiate withdrawal symptoms The lawsuit says she was placed on “special watch” by Wellpath staff that required her to be physically monitored every 15 minutes for any signs of drug withdrawal The lawsuit alleges that jail and Wellpath personnel “failed to evaluate” Niki every 15 minutes as required The family had told NPR that Niki’s addiction stemmed from being prescribed painkillers for several years to manage the pain from an injury When the doctors stopped prescribing those painkillers they said Niki then sought other ways to alleviate her pain Niki allegedly visited the jail’s medical department several times during the roughly three days she was in the jail Niki’s medicine administration chart indicated that she was suffering from nausea and vomiting and given the anti-nausea medication meclizine by Wellpath personnel Niki underwent additional medical screenings the following afternoon and was noted as appearing to be detoxing which resulted in the administration of an unspecified detox medication Niki allegedly called her daughter and reported experiencing chest at which time her daughter told Niki to seek help via the intercom button in her cell The complaint then claims her daughter overheard someone say Wellpath personnel allegedly listed Niki as having “moderate to severe” withdrawal symptoms [and] moderate tremors,” among other symptoms The lawsuit also alleges that Niki was given buprenorphine for withdrawal despite telling staff that the drug makes her sick She went to the medical department again for vomiting and other withdrawal symptoms Niki’s county-administered autopsy claimed that she died from “acute drug intoxication including fentanyl and diazepam,” but it did not list her as dying in the jail facility and other harm “in an amount to be determined at trial,” as well as attorney’s fees and other monetary relief In a statement about the lawsuit issued in July 2024 Orange County attorney Richard Golden said “Orange County disputes the allegations in the Capaci complaint and will vigorously defend the litigation on behalf of the named corrections officer and the County.” The Capaci lawsuit is just one of several filed against Wellpath over the years A 2019 investigative report by CNN found that the company had been sued for more than 70 inmate deaths between 2014 and 2018 Wellpath says it has operations in 37 states across the U.S. “serving more than 220,000 patients daily in more than 400 facilities the Board and management team determined that a court-supervised process to facilitate a sale of our Recovery Solutions and a separate reorganization of Wellpath Correctional Healthcare is the best path forward to strengthen our financial foundation and stabilize Wellpath for the long-term This action will enable us to continue providing for our patients and team members as we deliver essential services to vulnerable populations...” WHITE PLAINS - The family of a woman who died while being held at the Orange County jail last year is suing the county and its medical contractor at the jail for wrongful death The lawsuit on behalf of the estate of Niki Capaci was recently filed in federal court by her sister and administrator The primary defendants named are the county and Wellpath a limited liability company based in Tennessee with whom the county contracted to run the medical operations at the jail Niki Capaci was arrested by the Orange County Sheriff's Office on May 3 2023 for violation of the terms of her probation It does not say for what charge she was on probation She informed jail personnel she would require withdrawal management because she had recently used drugs The lawsuit documents the treatment she received over the next three days and alleges staff members were indifferent to her medical needs Medical staff at the jail began performing CPR and an emergency medical services crew that had been summoned took over that function when they arrived But the EMS crew eventually discontinued their efforts due to what the lawsuit calls "prolonged down time and obvious signs of death." an autopsy by the Orange County Medical Examiner's Office cited "acute drug intoxication" as the cause of death and her manner of death was "accident (substance abuse)." Restaurant inspections: How safe is your favorite place to eat in Orange County? But the lawsuit claims the medical examiner's office failed to properly evaluate the cause of Capaci's death because it failed to acknowledge the impact of one drug improperly administered to her and because of improper withdrawal management by the defendants the causes of action cited by the lawsuit are medical malpractice; negligence; negligent hiring training and supervision; deliberate indifference to serious medical need; and municipal liability The lawsuit seeks compensatory and punitive damages as well as pre- and post-judgment interest "Orange County disputes the allegations in the Capaci complaint and will vigorously defend the litigation on behalf of the named corrections officer and the county," Richard B said in a statement issued Friday afternoon Wellpath had not yet responded to a request for comment Friday Mike Randall covers breaking news for the Times Herald-Record and the Poughkeepsie Journal Niki Capaci, a mother of seven, died last year while incarcerated in a New York jail a top provider of medical care in that jail and hundreds of other facilities around the country alleging medical neglect in the death of their loved one But that case and dozens of other wrongful death, personal injury and medical negligence lawsuits filed by incarcerated individuals and their families are delayed because the multimillion-dollar company filed for bankruptcy last month "I'm more frustrated about the fact that I feel like [Wellpath is] kind of trying to shirk responsibility," said Layla Capaci "I don't care as much about my day in court or the money we f****d up.' And I feel like this is kind of a way for them to avoid having to do that." The company says that it will continue operating services in more than 400 facilities as it attempts to reduce approximately $550 million in debt and plans a reorganization of its Wellpath Correctional Healthcare division For years, Wellpath, the largest commercial provider of health care in jails and prisons across 37 states, has been the target of federal lawsuits and scrutiny by lawmakers for its practices that have been alleged to cause long-term health problems and the deaths of dozens of incarcerated individuals As part of the bankruptcy proceedings, a federal judge in Texas granted a pause in all lawsuits that involve Wellpath. Legal proceedings in such cases can take years in normal circumstances, but Wellpath's bankruptcy means dozens of those cases, like the Capaci case died while incarcerated in a New York jail cell in May 2023 some members of its staff and Wellpath in federal court for damages for wrongful death Capaci was exhibiting symptoms of severe withdrawal from opioids and was given buprenorphine a medication used to treat opioid use disorders even though she was intolerant to the medication That "only exacerbated her symptoms and contributed to her premature death," the lawsuit alleges A New York State Commission of Correction report has since agreed that jail staff failed to adequately evaluate Capaci that they failed to send her to a medical provider or to transfer her to an emergency room when her symptoms indicated the need for further care The report also found that corrections officers failed to properly monitor Capaci during their rounds The impact of the bankruptcy goes much deeper than just a delay to those involved in lawsuits against Wellpath Tens to hundreds of thousands of dollars of alleged damages are potentially being given up as some individuals look to drop claims against Wellpath And for others who turned to the courts as a way to receive justice they now wonder whether accountability will ever happen Representatives for Wellpath didn't immediately reply to NPR's request for comment Chief Executive Officer Ben Slocum said in a statement announcing the bankruptcy that the company continues "to put patient care first and made significant investments to ensure our patients received the highest level of care." Wellpath has been dogged by more than 1,400 lawsuits filed across the country. A 2019 CNN investigation found that in lawsuits filed between 2014 and 2018 It's unclear how many lawsuits or settlements naming Wellpath as a defendant are now directly impacted by this bankruptcy but court documents list at least around 30 cases tied to individuals in prison Bankruptcy has been a tactic used by corporations facing mass tort liability and it's a trend that appears to be growing a law professor at Temple University specializing in bankruptcy "It seems to be a more attractive path for companies to use to try to manage" liability brought by these civil cases Purdue Pharma, the maker of OxyContin, has attempted a similar maneuver as has correctional health care company Corizon. Taking this path "can give the company significant advantages through the bankruptcy process that they never have in ordinary litigation halting determinations of liability on the merits and many other things that can be quite valuable to the company but might be costly to victims," Lipson said It will cost the plaintiffs in more ways than one The filing forces alleged victims from the lawsuits to proceed with those unresolved complaints in bankruptcy court That could mean alleged victims could get far less in damages compared to what they could have received if the case continued in civil court "There's really no way that you can collect everything that you would otherwise be entitled to" once a company goes bankrupt the attorney representing the Capaci family the director of Gender Justice at Rights Behind Bars represents three clients with claims against Wellpath one yet to be filed and the other filed in December 2023 involve incarcerated individuals who are transgender who allege corrections staff and Wellpath were delaying access to necessary gender affirming care The needs Dangaran's clients have are urgent the ability to get those medical needs addressed is further delayed Dangaran said the advocacy group's clients now have two options: Drop claims against Wellpath and with it potentially hundreds of thousands of dollars and the chance to hold the company accountable for harms they allegedly faced or wait while critical health care continues to be allegedly withheld "You have to let [Wellpath] go or your entire case can be stayed to negotiate down their monetary awards in favor of guaranteeing gender affirming care," Dangaran said but the money potential is not as important as pursuing their care." Discovery in the Niki Capaci case should have started this winter but it has been delayed by the bankruptcy filing The Capaci family was hoping to get full access to a great deal of information — including the unredacted report from the New York State Commission of Correction Oh has filed a request to the judge to allow the case to continue against the other defendants in the Capaci lawsuit including jail staff and Orange County while Wellpath continues its bankruptcy proceedings A response from the judge is expected soon The family say they've dug in their heels and promise to continue with their case "I don't really think any of us planned on stopping anything until we got all of those changes and justice that my mom Mahwah-Rose Marie Capaci (nee Cariddi) of Ramsey 2023 at the age an early age he had a part-time job with a small residential land development company and recalls that suddenly “one day I knew I wanted my career to be in the construction industry.” He is not as certain about why he chose to prepare for that career at Arizona State University and the Del E Capaci got experience through student organizations and internships He served a term as president of the ASU chapter of the American Society of Civil Engineers (ASCE) and co-captained an ASCE-ASU Construction Solutions team for one of the major student competitions organized by the nationwide Associated Schools of Construction engineering and construction supply fields Among the most rewarding experiences of his time in college was an internship for which he spent a summer working in Anchorage Alaska working for DOWL construction and engineering company as a construction administration intern Another experience that had a big impact was learning from civil engineering Professor Barzin Mobasher “He taught me how to apply critical thinking to civil engineering analysis rather than simply approaching analysis and other problems in a cookie-cutter fashion,” Capaci says he will go to work for TITAN Formwork Systems in Tempe where he will design concrete formwork and shoring (support) systems for buildings and other structures he says he would like to be able to “3D print all the peanut butter I want.” Other than the career goal of using his work in construction engineering “to change the world for the better by making life easier for the general public,” he aspires to “own a bass boat.” wasn’t until the assassination of Italian judge Giovanni Falcone in 1992 that things took a sharp turn — pushing the Palermitans to fight against mob influence An example of this push-back is the ‘Spazi Capaci’ project recently organized by Fondazione Falcone. Established by the judge’s sister, Maria, the foundation teamed up with several artists to install public artworks across the city each one denouncing the Mafia while honoring its victims. ‘We’ve always known that culture is one of the best weapons against the Mafia,’ said Falcone You also need social and cultural work for that.’ mut-p53 dependent release of exosomes enriched for miR-1246 induces macrophage polarization towards pro-tumorigenic M2 phenotype further stoking tumor promoting inflammation suggesting that mutant-like p53 activities in non-transformed cells of the TME may have wider prevalence and stronger impact than initially appreciated Figure 2 Crosstalk between mut-p53 and HIF1α within tumor tissues mut-p53 sustains HIF1α function in both normoxic and hypoxic conditions mut-p53 interacts and inhibits the p63 tumor suppressor thereby abrogating SHARP1-mediated degradation of HIF1α HIF1α is stabilized by loss of VHL- dependent ubiquitination independently of p53 status In both conditions mut-p53 is able to bind and cooperate with HIF1α at the chromatin level to promote expression of hypoxia responsive genes and other tumor-promoting HIF1α target genes p53 and HIF1α are connected by a positive feedback loop and likely in other HIF1α stabilizing conditions HIF-1α upregulates the TP53 promoter wild-type p53 may undergo hypoxia-induced conformation changes acquiring a mutant-like (ml) phenotype Both mut-p53 and ml-p53 bind HIF-1α protecting it from degradation and chaperone HIF-1α to hypoxia-responsive elements (HREs) finally enhancing HIF1α transcriptional activity potentially influencing the activity of stromal and immune cell populations The effectiveness of the p53 tumor suppressor in providing a barrier against neoplastic transformation largely relies on its unique ability to act as a sensitive collector of stress inputs, both intrinsic and extrinsic to incipient tumor cells. Likewise, mut-p53 oncoproteins also respond to cancer-associated stress conditions, including microenvironmental cues (8) and hence their oncogenic activity becomes empowered by a tumor-supportive microenvironment mut-p53 triggers a mechano-stimulatory circuit that both generates and transduces mechanical cues within tumors Interestingly, these abilities are also transmissible to non-transformed cells: by promoting secretion of podocalyxin-rich exosomes, mut-p53 R273H and R175H proteins were found to modulate RCP/DGKα-dependent endosomal recycling in receiving normal fibroblasts that populate the TME of primary and secondary tumor sites, inducing their α5β1 integrin-dependent activation to a cancer associated (CAF) phenotype, increasing tumor invasiveness (64) All these evidences illustrate how mut-p53 by inducing ample structural modifications of the secretory trafficking machinery can amplify the range and intensity of the tumor cell secretome while also tuning the entire crosstalk of cancer cells with both surrounding and distant microenvironments missense TP53 mutations could endow cancer cells with genomic instability via retroelements-induced genome rearrangements while silencing the consequent suicide immune response mut-p53 can impinge on several aspects of the communication between cancer cells and immune-inflammatory cell populations of the TME thereby evoking tumor-supporting inflammation while concomitantly suppressing innate immune signaling and favoring immune evasion Elucidation of the role of mut-p53 as a regulator of the tumor-stroma crosstalk may offer several hubs for tailoring therapeutic approaches to treat tumors bearing TP53 mutations The evidence reported in this review suggests that components of mut-p53 induced secretome could represent ideal non-invasive biomarkers of prognosis and response to existing therapies as well as actionable targets for personalized treatments Interfering with selected secreted mediators might indeed inhibit the communication between tumors and host tissues Lastly, increased PD-L1 expression in mut-p53 positive lung cancer (82) may represent a valuable therapeutic window for use of anti–PD-1/PD-L1 immunotherapy the functional outcomes of mut-p53 dependent regulation of secretion and trafficking provide an ensemble of druggable processes which could be targeted by combination therapies we have summarized evidence of a bivalent interplay of mut-p53 with the TME mut-p53 senses inputs originated by tumor surroundings and promotes secretory pathway adaptations mut-p53 generates a multitude of output signals boosting their local and systemic delivery to non-transformed tissues Recent advances in our knowledge of the underlying molecular mechanisms has disclosed the importance of mut-p53-dependent secretome for enabling metastatic competence Similar to cell-autonomous activities of mut-p53 an open issue regards how much the effects on secretome composition may vary according to distinct p53 oncogenic variants and to specific tumor contexts it appears that the ability to induce structural alterations of the secretory machinery in tumor cells is conserved Many implications of these structural adaptations remain unexplored: for instance Golgi dysfunction induced by mut-p53 might cause mis-glycosylation of ECM components fostering tumor-promoting inflammation and immunosuppression Future research along these directions may disclose further layers of complexity in the effects of mut-p53 on the crosstalk between cancer cells and TME Exploring the impact of mut-p53 on tumor secretion holds great potential to extract biomarkers for prognosis and prediction of treatment response it is advisable that validation of markers and therapeutic targets will be actively pursued along the most promising lines of research with the recommendation that systematic studies are performed to compare different missense mut-p53 variants in multiple cancer types We project that these activities may provide effective approaches to blunt tumor aggressiveness and GDS collected and discussed the material All authors contributed to the March 15, 2012 at 1:15 am CDTExpandAnthony F. Capaci OREGON – The owner of a well-known Ogle County campground lost his life in a snowmobile crash in northern Wisconsin last week. Anthony F. Capaci, 39, of Elgin, died Friday when his snowmobile collided with a snowplow at 10:24 a.m. in Hurley, Wis., which borders the upper peninsula of Michigan. Capaci owned Lake LaDonna Family Campground on Harmony Road southwest of Oregon. He bought the campground from Lamont and Veronica Gaston in 2010. According to the website of the Wisconsin Department of Natural Resources, Capaci was traveling on a marked snowmobile trail in the city of Hurley, ran a stop sign and struck the snowplow. He was pronounced dead at Grand View Hospital, Iron County Sheriff Tony Furyk said. He released no further details. Several attempts to reach a spokesperson at Lake LaDonna were unsuccessful. Capaci’s funeral was Wednesday at Miller Funeral Home in West Dundee. Survivors include his wife, Stacie, and their three sons, Vincent, Julian, and Ethan.
