Microcephaly was noted in all, 4 (6.9%) children were born prematurely, but in 41 (70.7%) newborns, the birth weight was less than 3000 g, while the body length was less than 50 cm in 25 (43.1%) children. Decreased lymphocyte levels were observed in 64.8% of them (Table 4) The most common reductions were in absolute values of CD4+ and CD19+ (89.6%) the level of NK cells was more frequently elevated (62.5%) The most common reduction among immunoglobulins was in IgG concentration (72.9%) Immunoglobulin M was within the normal range in most cases ethical considerations must be taken into account especially when neurological impairments progress as congenital chromosomal fragility and radiosensitivity will remain unchanged even with a successful HSCT the decision to pursue transplantation should be considered individually for each patient taking into account the clinical severity of immune defects and other manifestations Immunoglobulin replacement therapy was administered to 37/63 (58.7%) NBS patients with low IgG levels and/or recurrent and severe infections Patients received regular immunoglobulin at a 400-800 mg/kg dose once every 28 days Regular prophylactic immunoglobulin replacement therapy became available in Ukraine only in 2006 immunoglobulin replacement therapy was episodic The median age of starting immunoglobulin therapy was 3.5 years Prophylactic antibiotic therapy was administered in 13/63 (20.6%) patients with recurrent respiratory symptoms Other treatment options included antiviral and antifungal therapy when necessary it was performed in remission of malignancies considering the complicated infectious history progressive decline in T- and B-lymphocytes the first experience of HSCT in Ukraine in a 3-year-old NBS patient was unsuccessful The patient did not have malignancy but had a severe recurrent infection and progressive T-cell lymphopenia A compatible related donor was not available With the help of the Ukrainian bone marrow donor registry a compatible non-relative donor (9/10 match at the A locus) was found in the International bone marrow donor registry After comprehensive pre-transplant evaluation the patient underwent reduced-intensity conditioning (RIC) according to the EBMT/ESID Guidelines for HSCT for primary immunodeficiency (modified preparative regimen for patients with radiosensitivity) Engraftment was achieved within the expected timeframes: neutrophil on day +11 Molecular genetic study of donor chimerism in peripheral blood by PCR on days +27 and +197 confirmed complete donor chimerism - 100% Starting from the early post-transplant period the patient experienced immune complications: graft-versus-host disease (GVHD) which was resistant to glucocorticoid therapy vedolizumab also did not have a positive effect The patient developed gastrointestinal bleeding and multi-organ failure progressed The child died on day 253 after transplantation The diagnosis of NBS in Ukraine began in 1998-2001 at the Department of Hematology of the Lviv Regional Children’s Specialized Hospital (now Western Ukrainian Specialized Children’s Medical Center) (22, 23). Molecular diagnosis for the first patients was conducted at the Institute of Medical and Human Genetics Charité, Berlin, Germany (3, 7, 24) The molecular-genetic diagnosis of NBS in Ukraine began in 2004 at the Institute of Hereditary Pathology Professor Larisa Kostyuchenko played a significant role in the study, diagnosis, and treatment of NBS patients in Ukraine (18, 23, 25). The first cohort of patients (1999-2007) were described by Dr. Kostyuchenko in 2009 (18) The prevalence of NBS patients with the Slavic variant in the western regions of Ukraine as of 2017 was 3.6 per 1,000,000, reaching up to 24 per 1,000,000 in some areas of this region (4), one of the highest in Europe and the world. The highest prevalence of NBS was reported in Poland and the Czech Republic, where it was 3.1 per 1,000,000 (19, 24) although there is a steady trend towards improved diagnosis in central regions NBS was diagnosed in two children due to positive results of newborn screening for SCID confirmed by genetic analysis Wolska-Kuśnierz et al. (19) 2015 described 149 NBS patients with the Slavic variant of NBN from the European Registry for PID among which the majority were patients from Poland (118) and Ukraine (14) the median age of NBS patients in our cohort is slightly lower than in the mentioned study (11 versus 14.3 years) The percentage of deceased patients in our study is slightly lower (33.3% versus 39%) but the children died at a younger age (median age of deceased 9 years versus 11.1 years) Overall, 6.9% of patients were born prematurely, but 70.7% had low birth weight (less than 3000g), while a body length of less than 50 cm was observed in 43.1% of children. Our results coincide with the data described in previous studies (6, 19). Although there were no premature births in the previous cohort, 42.9% of mothers experienced spontaneous abortions during previous pregnancies (18) Data reflecting the dynamics of physical development in NBS are limited. We have shown that up to the age of 10, most patients lag in weight, and a significant portion (from 28.6% to 50%) in body length/height. Another study also showed significantly lower anthropometric indicators in NBS patients than in the healthy population (11). As in our observations, Varon et al. (6) also note that growth deficiency improves with age The improvement in the physical condition of children with NBS may be associated with better diagnosis as well as improvements in socio-economic status and associated conditions that arise with it Microcephaly was observed in all patients with a median deviation in head circumference of -6.5z at 1 year Autoimmune and allergic manifestations are also characteristic of IEI (31, 32), although they were less common in NBS patients (15.5% and 8.6%, respectively), which aligns with reports from other researchers (19) Changes in humoral immunity were most commonly reflected in decreased levels of IgG (72.9%) and IgA (56.3%), which also corresponds to the data from previous studies (62% and 57%, respectively) (19). The authors noted a correlation between the degree of IgG and IgA deficiency and the severity and/or chronicity of respiratory tract infections (19) Implementing newborn screening for T- and B-cell lymphopenia may also help improve early diagnosis of NBS as well as careful registration and vigilance regarding patients with microcephaly Prenatal diagnosis is important for families with cases of children’s death at an early age from severe or recurrent infections or malignancies and the presence of individuals in the family with microcephaly Frequent miscarriages should also raise suspicion of NBS Considering the high frequency of the 657del5 variant of the NBN gene predictive genetic testing may be considered Early diagnosis will allow for appropriate prevention of infections avoiding radiation exposure for diagnosing infections and other pathologies and selecting the appropriate strategy for treating malignancies A noteworthy aspect of this study is that it is the first comprehensive analysis of the prevalence and immunological characteristics of patients from various regions of Ukraine over 25 years Previous studies mainly focused on data from patients in western regions during the initial 10 years of diagnosis or the prevalence of malignancies was limitedly studied The cohort of Ukrainian patients with Slavic variant in the NBN gene is one of the largest in the world which enhances the generalizability of the findings The analysis was conducted on the growth and weight parameters of children with NBS from birth to 18 years which has been insufficiently addressed in previous publications a limitation of this study is its retrospective data collection which posed challenges in obtaining certain data as well as the possibility of missing data or inaccuracies in the medical records and conducting a thorough assessment of result completeness Transitioning to adult care resulted in worsened patient monitoring Another limitation of this study is that we identified the c.657_661del5 variant of the NBN gene while around 10% of patients with NBS harbor alternative pathogenic variants which could influence the epidemiological results The prevalence of NBS in the Ukrainian population may be higher with improved diagnosis and surveillance analyzing data longitudinally allows for identifying diagnostic trends and determining ways to NIJMEGEN, Netherlands — Hundreds of U.S. military personnel joined with service members from 34 different nations to complete the 106th edition of the world’s largest multi-day marching event on Friday. The 560 U.S. entrants were among more than 6,400 service members and 40,000 others who marched approximately 100 miles in and around the Dutch city of Nijmegen during the Vierdaagse, or Four Days Marches. One group of service members raised eyebrows from day one. Seven guardians from the 53rd Space Operations Squadron in Landstuhl, Germany, gave Space Force their first team in an event that originated with about 300 Dutch soldiers looking to build stamina in 1909. The guardians enlisted the help of soldiers from Kaiserslautern and Wiesbaden to reach the 12-member team requirement. “Every day has been ‘Space Force?’ and lots of ‘Wait, that’s real?’ so the team has been fielding questions multiple times a day,” Senior Master Sgt. Robert Baer said Thursday. “It’s always great to get the Space Force out there in front of people, because it’s still so new.” The Americans said camaraderie between the international militaries and the hospitality of the Dutch people were the things that stuck out the most over the four days. “Being with all the partner nations and seeing how they do things while being able to exchange patches and shirts with all of them ... it’s great,” U.S. Army Sgt. 1st Class Edward French, who is stationed in Garmisch-Partenkirchen, Germany, said around the halfway point of Friday’s march. “The city, the participation that you see from all the kids and the high fives, you can’t beat it,” French added. In the town of Gassel, a band set up and residents lined the streets before 8 a.m. to hand out snacks and greet marchers as they passed through. “There’s been a couple of towns where we’re marching next to Dutch soldiers and they’re cheering for the Americans. They really care for us just as much as we care for them,” Sgt. 1st Class Kerstan Harrivel said Friday at a military rest area outside of Overasselt. Harrivel, who participated for the third time, was the U.S. Army Europe and Africa team captain. She said her team prepared by logging over 1,000 miles since January and completing other long-distance marches across Europe. Many marchers said temperatures and humidity posed an obstacle this year. Though not as hot as 2006, which saw the event canceled after the first day, the weather caused officials to modify Friday’s final march. Civilian routes were reduced a little over 6 miles and military marchers could complete their roughly 25 miles without a 22-pound rucksack. Carrying extra weight aside, some of the military participants said the march takes a big toll on the body. “A lot of military members come in with the notion that ‘I’m in the military, so I’m in good physical condition,’” U.S. liaison support officer Marine Lt. Matthew Berthinet said Thursday. “It’s really an event that taxes different parts of your body than you’re used to working out.” Berthinet, who marched in 2022, added that preparing for the event is a time commitment with no shortcuts when it comes to putting lots of miles on your body and feet. Not everyone on the Space Force team made it to the Charlemagne rest area on Friday to earn their Vierdaagsekruis, or Four Days Cross medal. A few marchers were dropped for medical disqualifications through the first three days. But Baer said this shouldn’t be the last time people see the Space Force in Nijmegen. “There are enough guardians out there, so there’s no reason we can’t put together a recurring Space Force team,” Baer said after arriving at Charlemagne on Friday. “We’re gonna make it a thing.” After a short rest, marchers wearing their newly earned medals walked the final 3-mile stretch down Sint Annasstraat, known as the Via Gladiola, into downtown Nijmegen. two-day festival reflects on the concepts of peace and freedom through the 'Peace by Peace' event The celebration is part of the longer program of '80 years of Freedom' Commemorating these events and making history more accessible to the broader public is important to many historians Most people have probably heard about and even participated in commemorative activities surrounding the Second World War be it by attending a traditional ceremony or through engaging with monuments and museums But not everyone is (actively) involved in fostering commemorative practices and making this history accessible Yet some have made it their priority to engage with this topic has engaged with commemorative practices and the ideas of liberation and freedom in the Nijmegen region since moving there in 2012 ‘The historical context of the border region particularly the Liberation Route (international remembrance trail that connects the main regions along the advance of the Western Allied Forces toward the liberation of Europe inspired me to become actively involved in bridging communities and reinforcing the importance of memory’ the American Studies professor has organised performative acts and taught about the cultural and transnational aspects of liberation the importance of commemoration stems from the complex emotions that surround the aftermath of war – out of profound sadness we can still find ways to celebrate life and joy,’ says Mehring While many European nations have commemorations for the events of World War Two The bombing of the city in February 1944 by American bombers was the deadliest on Dutch soil (763 dead ed.) and the liberation efforts by the Allies have gone down in history thanks to movies like A Bridge Too Far the Allied bombing was particularly devastating – not just because of the physical destruction but also because of the emotional and psychological trauma it caused Operation Market Garden further underscores the region’s pivotal role in the war leaving deep scars on the landscape and in the collective memory of the people is remembered as the most courageous and heroic actions of the Liberation of the Netherlands These events have shaped the memory culture of Nijmegen and emphasise the necessity of remembrance.’ ‘The Sunset March is a particularly important opportunity for international students to reflect on war reaching beyond classroom readings and discussions’ ‘During the RUDESA Spring Academy (a collaboration between Radboud University and University of Duisburg-Essen ed.) students from the Netherlands and Germany can experience this together and examine how the transatlantic memory culture manifests.’ ‘The aim behind the mural is to bring forth the civilian and human perspective of September 1944 collaborated with Radboud historian Joost Rosendaal and based this mural on diaries from people who lived in Nijmegen during that period the mural does not only showcase the devastation of this military operation children handing out apples to American soldiers immediately after the liberation of Nijmegen and a nun going around on her bike and helping the wounded bring out a very different dimension of the otherwise well-known event,’ explains Manders ‘Cross-border cultural exchanges have become integral to the healing process’ Commemoration is not only about the locals remembering the events He believes that it should also serve as a bridge between people that transcends national and ideological boundaries He also sees this happening in Nijmegen: ‘Cross-border cultural exchanges between the Netherlands and Germany have become integral to the healing process (after World War Two ed.) and it is through such transnational frameworks that we build lasting peace songs like Lili Marleen and We’ll Meet Again are striking examples of remembering the human stories behind the conflict They are speaking to universal themes of longing for peace Through creating these transnational links and connecting broader communities in remembrance we can build lasting peace.’ Commemoration of liberation is also an important way to learn from history and prevent future conflicts ‘Commemoration should teach us to be resilient against political ideologies that promote aggression This remains important also for younger generations who grew up without direct memories of World War Two – Mehring being one of them: ‘I grew up during a time of optimism in Germany and I recognize that it is easy to think that democracy and freedom are a given But freedom must be actively lived and practised – it is not something that comes automatically Commemoration offers a means for me and younger generations to engage with the realities of war and the fragility of peace reminding us all that democracy is a responsibility Manders thinks the Waalpaintings are a way to ensure that these stories and lessons of the past stay alive ‘The mural of Operation Market Garden is a way to bring awareness and also show that we do not want such tragedies to happen again.’ She believes that it is especially relevant to teach younger kids about these topics too: ‘Nowadays with all the political developments and polarization it is essential that we provide kids with tools to help them learn how to look at a specific event from different perspectives and understand the human aspects of war By including the Operation Market Garden mural in the curriculum it would be possible to show students the local experiences of World War Two and make the stories seem more alive and personal.’ Manders adds: ‘The graphic style of the mural was thus a purposeful choice for us It distances the violence to be appropriate for public space and younger kids but the choice of using slightly different stencils still brings forth the individuality of the people and stories depicted.’ Universities play an important role in commemoration the liberation too ‘They should work in a way to connect academic study and public engagement Through different transnational initiatives we can deepen students’ understanding of transatlantic memory culture and history by making the fight for our freedom up close and personal while fostering a sense of responsibility towards the future,’ Mehring explains ‘Not all forms of commemoration are productive – some can be overly sentimental or even politically charged’ academics can contribute their expertise and resources to bring to life the human histories and stories Mehring himself is working on several projects: ‘One of them is a bilingual book Faces of Liberation based on the Foundation Faces to the Graves where we aim to add a life story to each of the soldiers’ graves at the Canadian War Cemetery in Groesbeek This and other projects are ways to reflect on the past while also helping to foster a culture of remembrance across borders.’ Manders has a similar perspective: ‘We must bring academic research outside the university walls and generate discussions Projects like the Waalpaintings form a possible way to do that Not only can we bring more nuanced and inclusive historical narratives to the public but we can engage with the community – this brings so many different people together but also other community members who get involved we often talk with people on the street which brings up interesting stories and fosters conversation.’ While engagement with commemorative practices remains ever relevant it is also important to be aware that this must be done in a productive manner ‘We must strive for a balance between emotion and reason Not all forms of commemoration are productive – some can be overly sentimental or even politically charged Thoughtful commemoration should combine emotional resonance with a rational This led them to write a book that is now available in open access The abridged public version will be distributed around the city ‘You have to see the administration in those days as kind of oligarchs,’ explains Joris van den Tol ‘The mayor and council promoted their own economic interests and those of their families through administrative positions.’ Even though Nijmegen was landlocked the city did play an important role in colonialism ‘Nijmegen was the most important city of the most prominent region of the Dutch Republic This enabled the city to put forward chairs of committees in the States General and administrators for the VOC and WIC.’ The governors were administrators within the Dutch East India Company (VOC) and the Dutch West India Company (WIC) acting as a link between the States General and these trade organisations Lacking biographical information on the Nijmegen administrators the researchers delved into minutes and trade information records of sales of oxen and timber to the WIC and VOC ‘Wood was used for the ships and oxen were shipped to the plantations to power sugar mills,’ says Van den Tol ‘Oxen from Nijmegen were shipped to the plantations to power sugar mills’ Goods landing in the western Netherlands were transported to Germany via Nijmegen And the same applied vice versa for exports.’ ‘This made it possible for Nijmegen to develop a processing industry adding value to goods in order to re-export them for more money we find French refugees starting a cloth trade in Nijmegen and being helped by the municipality to child labour from orphanages.’ ‘There was some debate about it within the Republic especially during the Eighty Years’ War but we found no evidence of this in Nijmegen.’ Joris van den Tol: ‘On the contrary you find articles opposing the abolition of slavery and it is clear from the coverage that everyone knew what was happening in Suriname down to the last detail.’ Enslaved people were also to be found in Nijmegen They were brought to the city by people who moved to Nijmegen from the colonies Since slavery was banned in the Netherlands but it is questionable whether they knew that There are at least three known cases of enslaved people in Nijmegen One of them was a man called Sedin who came to Nijmegen from the Indonesian island of Madura as a servant of the Van der Zwaan family Renowned regent families such as the Singendonck, Sweers and Swaen profited from the economic activities surrounding slavery The researchers felt it was important to show this interconnectedness ‘This offers a different take on Nijmegen’s history one that will hopefully be taken further in future,’ says Coen van Galen The researchers’ book is full of examples of Nijmegen’s political and economic interests But also of Nijmegen residents who went off to try their luck in the colonies There even appears to have been a VOC ship named Nijmegen which sailed to Asia but was shipwrecked just before returning to the Netherlands The study also provided a geographical link to the university campus. Adam Jacob Smits (1685-1742) was alderman (administrator) in Nijmegen for almost 30 years and mayor twice. He was also a governor in the WIC and VOC on behalf of Gelderland. He invested in the companies and even managed to have one of the company’s ships, Heerlijkheid Horssen which he expanded with heathland and forest The area currently houses the campus and hospital The research study offers starting points for much follow-up research as well as the role of the nobility on the whole He concludes: ‘And I am of course curious to hear the city historians’ response to this book.’ The research study was handed over to Mayor Hubert Bruls on Wednesday evening, 19 March. ‘The study’s conclusions are poignant. and Goffert station is designed to connect the vibrant energy of Nijmegen with the tranquility of nature in nearby Goffertpark Its excellent location makes the complex highly accessible offering the ideal balance between active sport and green relaxation As part of the broader master plan “Winkelsteeg,” it sets the tone for Nijmegen’s urban transformation towards 2035 aiming to strengthen the connections between surrounding neighborhoods and landscape areas For this unique development in sports and housing in Nijmegen Under the leadership of Burgland Bouw as the municipality’s contract partner the sports building and parking solution are being developed Together with Hellebrekers and Slangen+Koenis Architects while Slangen+Koenis Architects is responsible for the integrated total design Along with the partners Orange Architects and Felixx Landscape Architects & Planners they have developed the concept for the entire plot Our team has created a holistic vision that gives Nijmegen a sustainable and future-proof impulse for the area Our shared goal is to create an integrated sports and residential complex that perfectly suits Nijmegen’s context The design combines varied building heights and dynamic shapes to create a smooth transition to the surrounding nature A 60-metertall building serves as a striking landmark forming a gateway to the Winkelsteeg neighborhood alongside the adjacent “52 Degrees” tower The gradual height build-up allows the complex to seamlessly blend into its surroundings The result is a balanced and welcoming architecture that creates a pleasant living environment for residents The grand 50m competition pool is clearly designed and puts the focus on swimming The materialisation creates a warm and inviting atmosphere with views of the green hills through the windows Nijmegen’s rich geological history guides the aesthetic choices of the design Inspired by the local moraines with the sand deposits of the last ice age the volume and facade materialization reflect the structures and different layers of the subsoil the exterior subtly references the soil layers that form Nijmegen’s landscape This natural inspiration reinforces the bond between the building and its surroundings with an extensive network of green spaces that contribute to biodiversity and a pleasant residential and living climate and green hills around the building create a continuous interaction between the city and nature This green design offers space for relaxation and recreation while also serving functional purposes such as rainwater buffering and temperature regulation This sustainable approach not only supports climate goals but also enhances the livability and identity of the complex and its specific landscape surroundings and recreation within a landscapeoriented environment Living spaces are designed with attention to daylight and energy efficiency and the sports center offers high-quality facilities for sports events Special attention has been given to the design of sports halls and swimming pools and optimal acoustics create an inviting and functional space The central restaurant provides a vibrant meeting point where visitors can enjoy views of both the sports facilities and the green surroundings.The main entrance from up close with a spacious entrance square The parking system is an essential part of the design with an underground parking garage allowing above-ground space for nature The efficient parking facility and well-organized traffic flows make the complex accessible and user-friendly The entrances for cars and bicycles are strategically placed making navigation within and around the complex intuitive and easy which contributes to a pleasant user experience Between the main entrance of the sports centre and the residential tower is a spacious ‘pocket park’ that slopes upwards towards the restaurant The transparent end façade invites you to come in for a drink The building design emphasizes sustainability and climate adaptation Using passive climate strategies such as green roofs and strategically placed vegetation the building remains cool with efficient use of energy Rainwater collection and infiltration support resilient water management while the green surroundings improve air quality and the living environment The smart layout of functions and use of sustainable materials make the complex energy-efficient and future-proof In addition to plantings for aesthetic and functional purposes the design also supports an ecosystem for animals such as birds Diverse plant species create an appealing habitat that supports healthy biodiversity Birds and insects find natural nesting areas and food within the complex while nesting boxes and other measures enriches the local fauna This ecological design promotes sustainable harmony between the city and nature The interior design combines warm earthy tones with wood accents creating a calming and natural atmosphere that harmonizes with the surrounding nature With subtle accent colours and carefully chosen materials the interior evokes a sense of peace and harmony enhancing the experience for users and
