She was convicted of murder, imprisoned, an appeal, which ultimately led to her release five years later, and she had not coped well once released.” Cox said Hyde “fell in with the wrong crowd” and took drugs and alcohol to excess “to cope with what happened in her earlier life”. The court was told Hyde moved to the remote village of Mullion, on the Lizard peninsula in Cornwall, having suffered domestic abuse and violence. She developed a significant cocaine and alcohol problem and was diagnosed with bulimia. A postmortem concluded this had caused her death aged 32 in April 2024. She became an alcoholic and also used heroin and cocaine and would be injected with drugs by others. Stacey Hyde, also known as Anastasia Darlison, with her mum Diane Hyde outside court in May 2015 after being cleared of murder (Image: PA)Diagnosed with PTSD in prison, Ms Hyde had a history of bulimia nervosa, hepatitis C, kidney failure and drinking alcohol to excess. Hoping for a better life, Ms Hyde moved first to St Austell before settling in Mullion near Helston. In a moving tribute read out in court on her behalf and also on behalf of Stacey's mum Diane, her aunt Julie Hyde, said: "At 17 she killed a man and was convicted of murder and was sentenced to spend a minimum of eight years in prison. "After a campaign for her release, she appealed and was found not guilty of murder due to self-defence and was immediately released. She had spent five years in prison and she didn't cope well after her release and turned to a life of prostitution, self-harm, drugs and alcohol. She suffered from bulimia too." Mrs Hyde said she and her niece had been very close despite the geographical distance as she lived in Greece. She told the inquest that when Stacey was abroad and came to visit she was fine, but when she was in the UK "her life was a train wreck". She added: "She was the sweetest, kindest, most generous and funniest person I knew. Stacey didn't want to die. She just didn't know how to live." Mrs Hyde also told the hearing that her niece had planned to come out to Greece later in the year to spend some time with her, sort her life out, travel and write a book. She added: "She was clearly looking to the future." In 2017, two years after she was cleared of murder, Ms Hyde disappeared from the Taunton area in Somerset, prompting a police appeal to help locate her following growing concerns for her safety. The inquest heard that because of her bulimia nervosa, Ms Hyde had a history of electrolyte disturbance in her body from bouts of vomiting. As a result, she was admitted to hospital on 11 occasions in the year prior to her death on April 11, 2024. She also suffered two cardiac arrests but was saved on both occasions. On the day of her death, following a consultation with her GP in Mullion she was taken to the Royal Cornwall Hospital at Treliske in Truro following 48 hours of vomiting and not being able to keep fluids down. Blood tests revealed that her electrolyte levels were all over the place, including potassium, which can lead to nausea, collapsing and cardiac arrest. The inquest heard that a couple of hours after her admission to A&E she was found unresponsive but despite 30 minutes of CPR and resuscitation work by hospital staff she couldn't be brought back and was declared dead. The medical cause of death was given as bulimia nervosa. The inquest was told that Ms Hyde had been well known to Cornwall's mental health service teams for several years. Concluding in a death from natural causes, senior coroner for Cornwall and the Isles of Scilly, Andrew Cox, said Stacey Hyde had been a young woman who had an incredibly difficult life, who went to prison for murder, appealed against her conviction and was released after a retrial. He added: "She didn't cope well after being released and fell in with the wrong crowd and took drugs and alcohol as a way to cope with what happened in her early life. Alcohol or drugs were not responsible for her death neither was suicide. In fact she had been looking forward to going abroad to go and visit her aunt in Greece. As such this a death from natural causes." The mother of a Cork man who drowned in the river Lee on Wednesday evening has condemned the behaviour of people who filmed the emergency services recovering his body after doing nothing to assist him when he got into difficulty. Luke Hyde (34), from Wolfe Tone Street in Cork, drowned after he got into difficulties while swimming across the north channel of the river Lee from Pope’s Quay to Lavitt’s Quay at around 6.45pm on Wednesday evening. The incident was filmed by onlookers on their phones, with some live-streaming the tragedy. Mr Hyde’s mother, Lily Hyde, rang in to the Neil Prendeville Show on Cork’s Red FM to express her disgust that people could stand on the quayside and film her son drown and not come to his aid and then film his body being removed from the river. Ms Hyde told how she heard the Irish Coast Guard helicopter overhead as it swept up and down the river Lee, so she rang her son’s mobile to know if he was okay, only for it to be answered by a garda. “Luke ... was my baby son. I was disgusted when I heard you talking this morning about those people down there. It was like a circus, watching my son drown, instead of trying to help him,” said Ms Hyde, who lost another son, Brian, when he died in his sleep in 2019. “When I got down, they were putting him into the ambulance and I saw the crowds, I didn’t know what was going on. I said I need to hold my boy, I need to see him. I don’t know what his last thought or words were. I was thinking, ‘Did he call my name?’” “What have people in this world come to? Morons, I don’t think there’s even a word to describe them. It will never leave me, and the clips I saw on the news ... I don’t know how I’ll ever, ever forget it. I never will, it will live with me for evermore.” Taoiseach Micheál Martin extended his sympathies to the family of Mr Hyde and also criticised onlookers’ behaviour. “It’s a very sad and regrettable feature of modern life. We have mobile phones and devices. It was a horrendous what happened, and my understanding is two fellows went in for a swim, both got into difficulty, and one was able to get out and raise the alarm. “I read what the fire officer said, that they found it very difficult to comprehend that there were no lifebuoys thrown in to help, but people were taking photographs or videoing and for family members, it’s very traumatic, and it’s very, very sad.” Cork City Fire Brigade second officer Victor Shine said he hoped the tragedy would bring home to people the importance of assisting in such situations and not just adding to a family’s pain by filming it. “There were hundreds of people down there watching – they were a couple of people deep up along Pope’s Quay, across the Shandon Footbridge and down Lavitt’s Quay. There are three lifebuoys on either quay, and only one person threw one in to help the first guy. “It’s awful but hopefully it will open people’s eyes as to the consequences of such behaviour. in many ways epitomises the sense that the old guard won’t – or perhaps can’t – see that they are going to have to find completely different ways of talking to people “Voters are still rightly furious investigation and has taken action to make improvements in these key areas The investigation was prompted by the landlord’s 10 severe maladministration findings and a high overall maladministration rate including in areas such as repairs (94%) and damp and mould (100%) These included 23 findings of severe maladministration (10 relating to property condition There was an overall maladministration rate of 82% The Ombudsman put forward 548 orders to put things right for residents in these cases The report sets out 3 key areas for the landlord to improve in and a series of recommendations to help it achieve that Barriers to the complaints process still existed in policy and practice and the report found that the intention of the Code had been skewed in the landlord’s policy towards its choice and control There was a failure to escalate complaints with the Ombudsman intervening in 25 out of 44 cases to ask the landlord to accept or progress the complaint The evidence suggested that many of the issues resulted from historical under resourcing of its complaints team and complaints being held whilst delayed repairs were taking place Poor systems and knowledge and information management also contributed at times the landlord provided inadequate redress It is encouraging that the landlord has invested in knowledge systems training and has recruited a complaint quality and improvement officer the report recommended the landlord ensures its complaints policy is in line with the Code and it takes learning from the Ombudsman’s determinations seriously to identify the root cause of complaints There were a significant amount of repair cases with unreasonable delays mainly caused by poor knowledge exchange with repairs contractors missed or otherwise ineffective appointments and failure to effectively manage or oversee contractors’ work on individual repair jobs This was further exacerbated during the coronavirus pandemic when the landlord struggled to keep up as delivering compliant repair became harder There were also issues with the landlord’s response to reports of leaks Other issues in this theme included poor communication and failure to assist the resident with an insurance claim The landlord identified its need to reduce repairs times and has worked on this The report recommends that the landlord reviews the causes of its wasted appointments and create an action plan seeking to minimise these as well as update its damp and mould policies and procedures The landlord experiences peaks in service charge enquiries twice per year in response to it sending out information at set times It appeared enquiries were placed in a queue for response and answered in turn without any triage for common queries or process or criteria for prioritising responses This had historically resulted in residents typically waiting 5 or 6 months for a response but sometimes up to a year these were not specific enough or did not cover all the points they needed to The landlord has undertaken a number of improvements in this area The report has recommended it makes its policies clear and consistent about when a service charge enquiry becomes a complaint including appropriate signposting to the complaints procedure as well as making sure its systems and processes are effective so that residents receive timely and sufficiently detailed answers to their enquiries There are also a range of cross-cutting themes identified as part of this investigation including knowledge and information management communication and taking vulnerabilities into account in addition to changes it has already made to improve services for residents it responded promptly and constructively to requests for information and and how it responded to residents’ enquiries about service charges The investigation was prompted by the landlord’s 10 severe maladministration findings and a high overall maladministration rate These residents in these cases the landlord inferred blame for the severity of a leak on a resident as they were away at the time the leak occurred The landlord disputed that their home was uninhabitable and whether a bed had been damaged by water It also did not offer appropriate compensation despite the resident dealing with the additional stress of having cancer the landlord failed to make a repair for nearly a year leaving a vulnerable resident with peeling wallpaper The resident was afraid to use the heating due to the issues the landlord failed to provide appropriate redress and the report found that the intention of the Code had been skewed in the landlord’s policy towards its choice and control the report recommended the landlord ensures its complaints policy is in line with the Code and it takes learning from the Ombudsman’s determinations seriously to identify the root cause of complaints and failure to effectively manage or oversee contractors’ work on individual repair jobs There were also issues with the landlord’s response to reports of leaks Hyde special investigation summary and full report said: “The timing of this investigation provides a valuable opportunity for the landlord to further establish an open positive complaint handling culture throughout what is a growing organisation “Our investigation shows that residents have experienced the same problems repeatedly over several years but the landlord was initially slow to recognise and respond to these “Problems within repairs and maintenance in particular the management and monitoring of contractors had consequences across other teams which meant residents received poor service from more than one part of the landlord The landlord was complacent about delays in numerous areas of its service Action the contractor has “walked away from its responsibilities” to fix a series of defects at the luxury flats scheme in London residents accuse the contractor of “stringing them along” over repairs needed at the complex which they claim will cost £35m to put right which overlooks Hyde Park and includes a £175m penthouse are problems with the ductwork and corroding pipes Residents claim the cost of repairs at the development has hit £35m Residents said: “Defects were discovered from 2014 onwards and for the past decade Laing O’Rourke has carried out ad hoc remedial work whilst refusing to agree to an appropriate settlement.” Details about the dispute became public when O’Rourke chief executive Cathal O’Rourke wrote to clients three weeks ago saying that one of its dormant companies Companies “While the parties engaged in settlement discussions LOR’s offers to carry out the remediation works came with unacceptable caveats and the compensation offered by LOR was insufficient to fund such works by a third party OHP suspects that during this period LOR was pursuing a deliberate strategy of stringing along OHP in the hope of delaying the time when it would have to pay damages or carry out remedial works leaving OHP with no option but to launch litigation in 2021.” They added: “Despite OHP seeking assurances regarding the financial health of LOR from LOR and its parent companies and LOR continuously defending and participating in these proceedings OHP was shocked and dismayed to receive a letter on 12 February 2025 indicating that LOR Group was not prepared to continue to fund the litigation and LOR plc’s subsequent announcement that it was placing its subsidiary in liquidation “This action can only be seen to be an attempt to avoid the very substantial judgment we expect
